尤文氏肉瘤平片、MRI诊断价值

摘　　要：目的:探讨平片、MRI对尤文氏肉瘤的诊断价值。材料与方法:回顾分析13例经活检、手术病理证实为尤文氏肉瘤患者的资料。结果:长管骨9例,髂骨2例,头颅1例,L4附件1例。平片:长管骨骨干较大范围骨质破坏4例,骨皮质局部增厚5例,骨膜反应9例,Codman's三角3例。髂骨骨质破坏伴软组织肿块2例。MRI:13例均有骨质破坏,为斑片状稍长T1、稍长T2信号,STIR为高信号。长管骨、髂骨病变处骨髓信号增高(STIR)。软组织肿块9例、5例伴出血坏死,STIR为混杂高信号。病变侵犯长管骨骨骺3例、脑实质1例、脊髓1例。结论:平片与MRI相互补充,可提高尤文氏肉瘤的影像诊断能力。Purpose：To discuss the diagnostic value of plain film,MRI for Ewing′s sarcoma. Materials and Methods： Records of 13 cases with Ewing′s sarcoma proved by biopsy,surgery and pathology were analyzed retrospectively. Results：Among 13 cases,9 were situated in long bone,2 in ilium,1 in head and attachment of fourth lumbar vertebra respectively.Plain film findings as follows：With long bone,extensiveness destruction of bone in 4,cortical thickening？in 5,periosteal reaction in 9,Codman′s triangle？in 3.With ilium,destruction of bone accompanied with soft tissue mass in 2.MRI findings as follows：destruction of bone were seen in all cases,displayed slightly hypointensity on T1WI,slightly hyperintense on T2WI and hyperintense on STIR.Marrow in pathological changes of long bone,ilium showed hyperintense on STIR.soft tissue masses were seen in 9,accompanied with hemorrhage and mortification in 5,with farraginous hyperintense on STIR.Lesions invaded epiphysis of long bone in 3,substance of head and spinal cord in 1 respectively. Conclusion：Plain film and MRI can supplement to each other,and improve diagnostic accuracy.
