榄香烯联合希罗达/奥沙利铂治疗进展期胃癌

摘　　要：目的评价榄香烯联合希罗达/奥沙利铂方案治疗进展期胃癌的临床疗效和不良反应。方法 61例进展期胃癌随机分为2组:实验组31例采用榄香烯+希罗达+奥沙利铂方案化疗;对照组30例采用奥沙利铂联合ELF(足叶乙甙+亚叶酸钙+5-氟尿嘧啶)方案,比较2组的疗效和不良反应发生情况。结果实验组完全缓解(CR)5例,部分缓解(PR)18例,总缓解率为74%,中位疾病进展时间为7.1个月,中位生存时间为11.4个月;主要毒副反应为白细胞减少,其中Ⅰ～Ⅲ度占40%;手足综合征,其中Ⅰ～Ⅱ度占22%。对照组CR3例,PR 14例,总缓解率为57%,中位疾病进展时间为5.9个月,中位生存时间为9.7个月;主要的不良反应为白细胞减少,其中Ⅰ～Ⅲ度占53%。2组无Ⅳ～Ⅴ度不良反应。实验组临床症状及生活质量显著改善,2组比较有显著差异(P<0.01)。实验组免疫指标NK细胞活性及CD3、CD4、CD8阳性细胞百分率,CD4/CD8比值显著升高,2组比较有显著性差异(P<0.05)。结论香烯联合希罗达/奥沙利铂联合化疗治疗进展期胃癌疗效确切,不良反应发生率低,可提升机体免疫力,提高患者生存质量和化疗耐受性,值得临床推广。Objective It is to evaluate the curative effect and side effect of elemene injection combined capecitabine plus oxaliplatin regimen in the treatment of patients with advanced gastric carcinoma. Methods Sixty-one patients with advanced gastric carcinoma were randomly divided into two groups： Test group with 31 cases were received elemene injection plus capecitabine and oxaliplatin regimen chemotherapy;Control group with 30 cases were received oxaliplatin plus calcium folinate and 5-Fu and etoposide regimen chemotherapy.The curative effect and side effect were compared between the two groups. Results In test group,the total response rate was 74%（5 complete response and 18 partial responses）;the median time to progression was 7.1 months and the median overall survival was 11.4 months;the most common side effect was leucopenia（40%） and hand-foot syndrome（22%）.In control group,the total response rate was 57%（3 complete responses and 14 partial responses）,the median time to progression was 5.9 months and the median overall survival was 9.7 months,the most common side effect was leucopenia（53%）.No grade IV and V side effects were observed in both groups.The life quality and clinical symptom of the patients in test group were improved more significantly than those in control group（P〈0.01）,the value of CD3,CD4,CD3/CD4 and NK in test group were significantly higher than in control group（P〈0.05）. Conclusion The elemene injection combined capecitabine plus oxaliplatin regimen shows good efficacy and acceptable safety in patients with advanced gastric cancer,it can enhance the immune function and improve the life quality.It＇s worthy to be recommended.
