早期肠内营养联合中药治疗胃癌术后患者12例  

Early Enteral Nutrition Combined with Traditional Chinese Medicine for Postoperative Gastric Carcinoma: A Report of 12 Cases

摘　　要：目的观察早期肠内营养联合中药对胃癌术后患者体液免疫及胃肠功能恢复的影响。方法将24例胃癌手术患者随机分成两组,治疗组先滴注健脾通里中药煎剂100 ml后再滴注瑞素500 ml,对照组先滴注9.0 g/L氯化钠注射液100 ml,再滴注瑞素500 ml,每天1次,连续7 d后检测免疫球蛋白IgAI、gMI、gG及胃泌素(gastrin,GAS)含量,并观察胃肠功能的恢复情况。结果治疗组血浆IgA、IgM明显升高,肛门排气、排便复常时间,胃管引流量,胃管放置时间明显短于对照组;对照组术后GAS显著下降,而治疗组无明显下降。结论胃癌术后早期应用中药并辅以肠内营养,能够提高患者的体液免疫功能,促进胃肠激素的分泌,促进胃肠功能的早日恢复。Objective To observe therapeutic effects of early enteral nutrition combined with traditional Chinese medicine on humoral immunity and the recovery of gastrointestinal function in postoperative gastric carcinoma patients.Methods Twenty-four patients with postoperative gastric carcinoma were randomly divided into two groups.Twelve patients in the treatment group were treated with Chinese medicine decoction of Jianpi Tongli 100 ml and Fresubin 500 ml,while another 12 patients in control group were treated with Sodium Chloride Injection 100 ml and Fresubin 500 ml.Both of the two groups were treated once daily.The contents of IgA,IgM,IgG and gastrin（GAS） were detected 7 days later,and the recovery of gastrointestinal function was also observed.Results The levels of IgA and IgM in treatment group increased significantly,and passage of gas by anus,the time of defecation returned to common,discharge volumn from gastric tube,and the time of remaining gastric tube were significantly shorter than those in control group.The levels of postoperative GAS in control group significantly decreased while there was no significant decrement in treatment group.Conclusion Early enteral nutrition combined with Chinese medicine in postoperative gastric carcinoma could increase the humoral immune function,promote the secretion of gut hormone and the recovery of gastrointestinal function.
