摘　　要：目的探讨共刺激分子B7-H1在直肠癌与癌旁组织中的表达及临床意义。方法运用免疫组织化学检测共刺激分子B7-H1在50例直肠癌与相应的癌旁组织中的表达，对两者之间的表达水平作相关性分析，并探讨癌组织及癌旁组织的表达与临床病理参数之间的关系。结果B7-H1在直肠癌和癌旁组织中表达的阳性率分别为66．0％（33／50），20．0％（10／50），两者比较差异有统计学意义（P〈0．01），但两者之间的表达评分呈正相关（r=0．435，P=0．002）。在直肠癌组织中B7一H1的表达与患者性别、年龄、肿瘤大小及分化程度之间差异无统计学意义（P〉0．05），而与TNM分期、淋巴结是否转移以及术后生存时间有关（P〈0．05）；在癌旁组织中B7-H1的表达仅与术后生存时间有关（P〈0．05），而与其他参数之间的相关性不明显（P〉0．05）。结论B7-H1在直肠癌和癌旁组织中的表达，可能与肿瘤的进展和侵袭相关，可以作为直肠癌患者预后的诊断标志。Objective To investigate the clinical significance of the expression of costimulatory molecule B7 - H1 in human rectal carcinoma and para - carcinoma tissues. Methods Immunohistochem- istry was used to detect the expression of B7 - H1 in 50 cases of rectal carcinoma and corresponding para - carcinoma tissues, and the correlation of the B7 - H1 expression in the carcinoma and the para - carcinoma tissues was also analyzed. The relationship of B7 - H1 expression in carcinoma and para - carcinoma tissues and the clinical parameters was examined subsequently. Results B7 - H1 was highly expressed in rectal carcinoma tissues, and the positive rate was 66.0% （33/50）, whereas in para - carcinoma tissues the positive expression rate was only 20.0% （10/50）. There existed significant difference in the B7 -H1 expression between the carcinoma and paracarcinoma tissues（ P 〈 0.01 ） ,and a positive correlation was revealed（r =0.435 ,P = 0. 002）. The expression level of B7 -H1 was not associated with the sex, age, tumor size, degree of differentiation in rectal carcinoma tissues （P 〉 0.05 ）, but it was related to TNM classification,lymph node metastasis and survival time （ P 〈 0.05）. In para - carcinoma tissues, the expression level of B7 -H1 was only associated with survival time（P 〈0.05 ） ,and there were no marked correlation between B7 - H1 expression and other clinical parameters （ P 〉 0.05 ）. Conclusion The expression of B7 - H1 in rectal carcinoma and para - carcinoma tissues may he related to the progression and invasion of rectal carcinoma. The detection for B7 - H1 expression in carcinoma and para - carcinoma tissues may help predict the prognosis of patients with rectal carcinoma.
