摘  要:目的:比较研究根管治疗后上颌单根前磨牙邻牙合面(MO)洞形经5种方法修复后的抗力,评价聚乙烯纤维修复牙体缺损对抗力的影响,寻找增强修复后抗力的有效方法。方法:60个完整离体上颌前磨牙分成6组。第1组为对照组,另外5组根管治疗后预备近中MO洞形,第2组用复合树脂直接充填;第3、4组分别用石英纤维桩和聚乙烯纤维制作桩,复合树脂充填;第5组在窝洞表面涂布流体树脂,然后将聚乙烯纤维颊舌向嵌入流体树脂中,光固化后复合树脂充填;第6组复合树脂充填后,在修复体牙合面颊、舌尖间预备一沟,并越过颊舌面,然后同第5组涂布流体树脂,嵌入聚乙烯纤维,树脂充填。各组试件经热循环10 000次后,置于INSTRON测试仪上测试断裂载荷,记录试件断裂时测试机读数及试样断裂模式。结果:对照组断裂载荷最高,第2组最低。第6组与对照组的断裂载荷无统计学差别(P=0.776),其余实验组均显著低于对照组,第3、4、5组3组间差别无统计学意义(P>0.05),且均低于第6组、高于第2组。第1、2组与第4、5、6组间,第3组与第5、6组间的断裂模式有统计学差别(P<0.05),其余组间均无统计学差别(P>0.05)。结论:聚乙烯纤维嵌入修复体上方或下方时,均能显著提高修复体的抗折能力,其中以嵌入修复体上方者最好。AIM: To evaluate the effect of five restoration techniques and polyethylene fiber on the fracture load and fracture mode of endodontically treated premolars with class Ⅱ cavities in vivo,and to provide some alternative for non-vital teeth with minor loss of coronal dentin.METHODS: 60 single-rooted premolars were divided into six groups.Group 1 remained untreated and served as control.The other five groups were endodontically treated and prepared with standardized MO cavities.Group 2 were restored with composite resin.Group 3 were restored with quartz fiber posts and direct composite.In group 4,the Ribbond post system and direct composite were used.In group 5 and 6,the polyethylene fibers were used under or over the MO composite restorations.Each specimen was loaded in an universal testing machine until fracture.Data were recorded and submitted to ANOVA and Tukey post hoc test.RESULTS: The mean load necessary to fracture the samples in each group was: G1: 1191.04±362.59a,G2: 578.81±117.87b,G3: 909.33±250.20c,G4: 831.56±220.16c,G5: 900.66±185.57 c,G6: 1161.31±184.26 a.The incidence of repairable fracture mode were: 20%a、20%a、50%ab、80%bc、100%c、100%c.Different superscript letters demonstrate significant difference between groups.CONCLUSION: The use of quartz fiber posts and polyethylene fibers in premolars with class Ⅱ cavities can significantly increase the fracture load.The fracture mode can also be significantly improved especially when the fibers were used over or under the MO composite restorations.
