摘 要:目的DNA定量鉴别乳腺导管非典型增生与原位癌、原位癌及浸润癌。方法对乳腺导管非典型增生(7例)、原位癌(15例)及浸润癌(18例)共计40例进行细胞涂片,Feulgen染色,全自动图像分析仪进行细胞核定量检测。结果乳腺导管非典型增生及原位癌均未见DNA异倍体,未出现五倍体(5c)细胞,两组间DNA指数及S期细胞比率有统计学差别(P<0.05)。浸润癌检测到大量DNA异倍体,出现五倍体(5c)细胞,DNA指数及S期细胞比率高于原位癌,但不具有统计学差别(P>0.05)。结论利用图像分析仪检测DNA含量、S期细胞比率对乳腺导管非典型增生与原位癌的鉴别、原位癌与浸润癌的鉴别具有一定的临床意义。Objective The aim of this study was to evaluate the value of quantitative measurement of DNA content in differential diagnosis of proliferative breast diseases.Methods Cytological smears from a total of 40 cases of atypical ductal hyperplasia(ADH,7 cases),ductal carcinoma in situ(DCIS,15 cases)and invasive carcinoma(INV,18 cases)were subjected to Feulgen staining and image cytometry for quantitative detection of nuclear DNA.Results The ADH and DCIS showed no DNA aneuploidy,and there were no 5c cells.The S phase fraction and DNA index of DCIS showed higher values than that in the ADH with a statistically significant difference(P0.05).INV showed DNA aneuploidy,and the presence of 5c cells.The S phase fraction and DNA index of INV were higher than that of DCIS,but the difference was statistically not significant.Conclusions Quantitative evaluation of DNA content by image cytometry and S-phase fraction may have potential significance in differential diagnosis of breast ADH and DCIS,DCIS and INV.
