摘 要:目的以完整牙列为对照组,探讨上颌骨缺损患者赝复体修复后咀嚼运动时脑血流变化的差异。方法选取上颌骨肿瘤手术后已行赝复体修复的患者30例为实验组,另选择30名完整牙列志愿者为对照组,受试者取平卧位,安静30min后行颞窗的大脑中动脉的经颅多普勒超声扫描检查,分别测定修复后组、对照组不咀嚼、空咀嚼5min、10min时大脑中动脉的收缩期峰流速、舒张期末峰流速、平均峰流速。测得的数据结果采用SPSS13.0统计软件进行重复测量数据的方差分析。结果修复组与对照组对收缩期峰流速、舒张期末峰流速和平均峰流速的影响均无统计学意义。结论赝复体修复后上颌骨缺损患者咀嚼运动时大脑中动脉血流量与完整牙列受试者无显著差异。上颌骨缺损患者中空式阻塞器型赝复体修复后咀嚼运动时大脑中动脉血管充盈度接近完整牙列受试者水平。Objective To investigate the changes in cerebral blood flow in patients with maxillary defect treated with prosthesis insertion. Methods Thirty patients with maxillary defect receiving obturator prosthesis insertion were enrolled with another 30 subjects without dentition defect as the control. The cerebral blood flow rate was recorded before and at 5 and 10 min during mastication,and the results were analyzed statistically. Results There was no significant difference in Vs,Vd or Vm between the two groups at the time points for measurement. Conclusion The blood supply by the middle cerebral artery is similar between the patients receiving obturator prosthesis insertion for maxillary defect and the subjects with full denture.
