摘  要:目的:探讨微血管多普勒超声(MDU)在颅内动脉瘤夹闭术中的应用价值。方法:对20例行颅内动脉瘤夹闭术患者进行术中监测,记录夹闭前后载瘤动脉近、远端的血流速度、搏动指数及音频,并进行比较。结果:MDU证实有3例(15%)动脉瘤夹完全阻断了载瘤动脉远端,6例(30%)造成载瘤动脉远端狭窄。较大动脉瘤及局部解剖关系复杂的动脉瘤夹闭后导致其远端动脉狭窄的几率较高。结论:MDU是一种简便、安全、快捷有效的监测技术,尤其是在巨大动脉瘤夹闭术中使用,可确保动脉瘤夹闭后其远端动脉血流通畅,避免因动脉瘤夹位置不当导致远端供血动脉损害。Objective:To investigate the value of intraoperative microvascular Doppler ultrasonography(MDU) in the intracranial aneurysm surgery.Methods:MDU was used in 20 patients with intracranial aneurysm who were undergone aneurysm clipping.The proximal and distal blood flow velocity were recorded before and after the clipping and were compared.The pulsatility index and Doppler audio were also recorded and compared before and after the aneurysm clipping.Results:MDU demonstrated 3 cases(15%)parent arteries occlusion and 6 cases(30%)parent arteries stenosis after clipping of the aneurysms.The incorrect clipping was corresponding with size of the aneurysms.Conclusion:MDU is easy,safe,fast and effective monitoring techniques,especially in the patients with giant aneurysm.It can avoid occlusion and stenosis of parent arteries during the intracranial aneurysm clipping.
