摘　　要：目的通过设立专职护士加强腹膜透析患者的教育和管理,提高腹膜透析患者的社会回归。方法由专职护士负责门诊腹膜透析患者包括体液控制、饮食指导、运动康复、心理辅导、并发症的预防、就诊指导等方面的教育和管理,观察管理前后腹膜透析患者社会回归及相关因素的变化。结果腹膜透析患者的社会回归程度明显好转,影响社会回归的相关因素如容量负荷问题、营养状态等得到明显改善。结论设专职护士加强腹膜透析患者的教育和管理,可有效促进腹膜透析患者社会回归,提高他们的生活质量。Objective To explore the impact of nurse care manager model on social rehabilitation in peritoneal dialysis （PD） outpatients.Methods The nurse care manager model was used in the nursing care of 152 PD outpatients including body fluid control,diet guidance,exercise rehabilitation,psychological aid,prevention of complications,visit guidance and other aspects of education and management.The social rehabilitation in the patients and its relative factors were observed before and after management.Results After the nurse care manager model,the social rehabilitation in the patients improved.The factors influencing social,such as volume load,nutritional status,significantly improved.Conclusion The nurse care manager model can enhance the education and management of PD outpatients,effectively promote their social rehabilitation and quality of life.
