局部亚低温联合依达拉奉治疗高血压脑出血微创血肿清除术后继发脑梗死33例疗效观察

摘　　要：目的观察局部亚低温联合依达拉奉治疗高血压脑出血微创血肿清除术后继发脑梗死的临床疗效。方法采用美国国立卫生研究院卒中量表(The NIH stroke scale,NIHSS)评分法评估三组治疗前及治疗后7d、14d、30d的临床疗效,采用多田公式计算三组治疗前及治疗后3d、7d、14d的缺血灶大小。结果治疗7d后,三组NIHSS评分差异无统计学意义(P>0.05);治疗14d后,联合治疗组缺血灶大小较单纯亚低温组明显减少(P<0.01),依达拉奉组缺血灶大小较单纯亚低温组也明显减少(P<0.05);但联合治疗组和依达拉奉组缺血灶大小减少差异依旧无统计学意义(P>0.05)。结论早期对高血压脑出血微创血肿清除术后继发脑梗死患者进行局部亚低温联合依达拉奉治疗,能显著促进患者神经功能的恢复,减小缺血灶体积,挽救缺血半暗区神经元,提高患者的生存质量,改善预后。Objective To determine the cooperative effect of local mild hypothermia therapy（LMH） and intravenous infusion of edaravone on the treatment of 33 patients with cerebral infarction after the micro-invasives surgery for hypertension-induced hemorrhage.Methods Clinical outcomes were evaluated by using the NIH stroke scale（NIHSS） at 7-day,14-day and 30-day after treatment.Ischemia volume was assessed quantitatively by Duo Tian formula based on CT scan at 3-day,7-day and 14-day after treatment.Results There was no significant difference regarding NIHSS scores in all three groups at 7-day after the treatment.Compared with LMH group,respectively,ischemia volumes in both cooperative treatment group and edaravone group were significantly decreased at 14-day after treatment（P0.01,P0.05）.However,there was no significant difference regarding ischemia volume between cooperative treatment group and edaravone group.Conclusion Combined LMH therapy and intravenous infusion of edaravone has a cumulative effect that enhances the overall outcome of treatment for cerebral infarction after the micro-invasives surgey for hypertension-induced hemorrhage at early stage.It is beneficial to the ischemia volume elimination and neurons survival in the ischemic penumbra region,and contributed to the neurofunctional rehabilitation and improvement of quality of daily life.
