摘  要:目的:研究二尖瓣口充盈力学的三维效应与瓣口狭窄和左房能量的关系。方法:应用牛顿力学定律把力学效应的强度维、时间维和空间维映射到立体坐标系中,按三维模型构筑三维力效应图并求得三维力(3DF);另按压力一容积势能理论求得左房总能量(-FE)和能量效率(EE);用二维多普勒超声对自然瓣、狭窄瓣和机械瓣的辩口冲力(F)、冲量(l)。充盈功(W)和3DF与瓣口面积(MVA)、TE和EE作对比性研究。结果:三种二尖瓣口1、W和3DF有显著的差别;3DF与MVA、Th和EE成较好的相关关系和分级定量关系。而F在换瓣前后无显著差异;随MVA下降呈现双向(先升后降)变化,与后三者相关性欠佳。结论:3DF较单纯F更可靠地反映瓣口狭窄充盈动力学和左房能量变化趋势。Objective:To study the associations between three-dimensianal effect of mitral filling force and valvular stenotic degrees and left atrial enerny and enerny efficiency. Methods: By applying the law of Newton mechanics, intensity-dimension, time-dimension and spacial dimension of force effect were put into stereo coordinate system and three-dimensional figure of force effect were drawed up and three-dimensional force(3DF)was calculated according to three-dimensional model,Total energy(TE)and energy efficiency(EE)of left atrium were calculated by the theory of pressure-volume potential energy,By 2-dimensional Doppler echocardiography impulse force(F),impulse(I),filling work(W)and 3DF in the matural,the stenotic and the mechanical valve were studied by comparison with mitral valve area(MVA),TE and EE,Results:I,W and 3DF of three kinds of different mitral valves had significant difference.There were better relatipnships and classificated quantitative associations between 3DF and MVA,TE as well as EE,but F had no significant defference between the stenotic valve group and the mechanical valve group and presented an double-direction changes of early ascending and late descending as MVA decreasing and were less correlated with the latter three indexes.Conclusion:3DF in comparison with F had a significant superiority for evaluating mitral valvular filling dynamics and changes of left atrial energy.
