老年痴呆患者脑脊液tau蛋白及Fas抗原变化的意义

摘　　要：目的　探讨Ａｌｚｈｅｉｍｅｒ 病（ ＡＤ） 患者脑脊液（ＣＳＦ） 中ｔａｕ 蛋白和细胞凋亡因子Ｆａｓ 抗原含量变化的临床意义　方法　采用夹心式酶联免疫吸附法（ＥＬＩＳＡ） 测定了２７ 例ＡＤ、１２ 例多发梗塞性痴呆（ ＭＩＤ） 和对照组１８ 例患者ＣＳＦ 中ｔａｕ 蛋白及Ｆａｓ 抗原含量。　结果　ＣＳＦ 中ｔａｕ 含量，ＡＤ 组为（４８６－７４６ ±３２８－９４０）ｐｇ·ｍｌ－１ ，明显高于ＭＩＤ 组的（１４８－６３４ ±１３３－３７７）ｐｇ·ｍｌ－１ 及对照组的（１０６－１５５ ±９５－４９２）ｐｇ·ｍｌ－１ ，ＡＤ 组与 ＭＩＤ 组间、ＡＤ 组与对照组间差异均有非常显著意义，ＭＩＤ 组与对照组间差异无显著意义；ＡＤ 组ＣＳＦ 中ｔａｕ 蛋白含量的增高与发病年龄没有相关关系，随着病情发展，ｔａｕ 蛋白值有升高趋势，但不同痴呆程度患者间ｔａｕ 蛋白值的差异无统计学意义。同时测得ＣＳＦ 中Ｆａｓ 抗原含量为ＡＤ 组（１１－１１９ ±３－０９５）ｎｇ·ｍｌ－１ ，稍低于ＭＩＤ 组的（１２－６６０ ±７－８９３）ｎｇ·ｍｌ －１及对照组的（１３－２７０ ±８－４９１）ｎｇ·ｍｌ－１ ，但３ 组间Ｆａｓ 抗原含量的差异及ＡＤ 组中不同年龄段或不同痴呆程度患者的Ｃ?study the tau protein and Fas antigen concentration in the cerebrospinal fluid (CSF)of patients with Alzheimers disease(AD)and evaluate its clinical significance. Methods By using the ELISA method to detect the changes of tau protein and Fas antigen concentration in CSF of 27 patients with AD,12 patients with multi infarct dementia(MID)and 18 controls with other diseases of central nerve system. Results CSF tau concentration was very significantly increased in AD as compared with MID and controls,but no significantly difference between MID and controls.The increase of CSF tau in AD had no correlation with age.As the disease developing,the tau value had an increase tendency,but no significantly difference among the patients of different dementia degrees.While,CSF Fas antigen concentration was slightly decreased in AD as compared with MID and controls,but no significant difference among all the patients of AD、MID and controls.Also,there was no significant difference among the CSF Fas antigen concentration of different ages and different dementia degrees of AD patients. Conclusions Increase of CSF tau concentration may be an important significance in the diagnosis and differential diagnosis of AD,but the changes of CSF Fas antigen concentration may be of no important significance in the diagnosis and differential diagnosis of AD.
