非要素经肠营养剂对进展期胃癌围手术期的营养支持

摘　　要：进展期胃癌病人，多伴有不同程度的营养不良和免疫功能受损。本实验旨在应用非要素经肠营养剂改善此类病人的营养状况和免疫功能，以提高其手术及术后放、化疗的耐受力。用经肠营养剂经口服或管饲对进展期胃癌病人围手术期进行营养支持，并设等热等氮的肠外营养组进行对照，观察血浆蛋白等营养指标及免疫球蛋白等免疫指标。结果表明，营养支持后，实验组病人营养指标的改善与对照组相近，但ＩｇＧ、ＩｇＡ、ＩｇＭ 等多项免疫指标明显优于对照组。说明用非要素经肠营养剂对进展期胃癌病人在围手术期进行营养支持，其疗效明显优于等热等氮的肠外营养支持。The patient with advanced gastric cancer is frequently companied with malnutrition and immune dysfunction by various degrees.We applied a new non elemental diet,which was searched by ourselves since 1996,to this kind of patients during preoperative and postoperative period to improve their nutrition status and immune function.The patients were divided into two groups:the control group and the experimental group.The patients in the experimental group were given non elemental diet by oral intake or tube feeding.Those in the control group were supported with parenteral nutrition which was equal to the experimental group in calories and nitrogen.The results showed that the immune data ssuch as IgG,IgA,IgM,C 3,C 4,CD + 3,CD + 4,CD + 8,CD + 4/CD + 8,NKC,TNF,and IL 2R in the experimental group were significantly superior to those in the control group.This study indicates that non elemental diet entral nutrition support applied to the advanced gastric cancer patients during preoperative and postoperative periods is significantly superior to parenteral nutrition support with equal calories and nitrogen.
