食管癌切除、食管胃弓上吻合148例体会

摘　　要：目的　探讨食管癌切除后，食管胃弓上吻合的手术技巧和方法。方法　对１４８ 例食管癌切除术后食管胃弓上器械吻合的临床资料进行认真复习，并对其操作特点进行分析。结果　１４８ 例食管癌全部手术切除，食管胃弓上器械吻合，发生吻合口瘘１ 例，急性胃扩张２ 例，其余痊愈出院。结论　食管癌切除后食管胃弓上器械吻合时，吻合口距主动脉上缘１５ —２ 厘米，避免吻合后成角，胸腔胃应缝缩３ —４ 针。Objective: To study the technique and method of esophagogastric anastomosis. Methods: The clinic date of 148 esophagogastic anastomosis were restudied and there clinic characteristic were analysis. Results: All of 148 patients of esophagus cancer were operated with esophagogastric anastomosis above aorta arch. They have recovered except 1 esophageal fistula and 2 acute gasterectasis. Conclusion: The distance of esophagogastric anastomosis is at least 1.5-2Cm from arch and avoiding have angle. The another point is made the stomach of introthoracic to stomach tube.
