经阴道及腹部彩色多普勒超声在子宫肌瘤与子宫腺肌瘤鉴别诊断中的应用

摘　　要：目的探讨子宫肌瘤和子宫腺肌瘤的二维图像及彩色血流分布特点,提高两种疾病的诊断率。方法对45例子宫肌瘤和38例子宫腺肌瘤患者行彩色多普勒超声(CDFI),分析其二维图像及彩色血流分布特点。随机选择40名健康妇女为正常对照组。结果子宫肌瘤组和子宫腺肌瘤组患者发病年龄及临床表现相近,但子宫腺肌瘤的痛经、子宫体触痛明显;二维图像上子宫肌瘤及子宫腺肌瘤均呈特征性改变。CDFI示子宫肌瘤及子宫腺肌瘤患者子宫动脉平均阻力指数(RI)低于正常对照组(P<0.05)。子宫肌瘤周围有环绕血流,浆膜下肌瘤及黏膜下肌瘤的蒂部显示丰富的血流;子宫腺肌瘤周边血流信号稀少,内部见丰富或星点状血流信号;频谱显示子宫腺肌瘤阻力指数高于子宫肌瘤周边及内部动脉RI。结论 经阴道及腹部超声对子宫肌瘤与子宫腺肌瘤的鉴别有重要价值,但必须结合临床表现才能提高二者诊断的准确率。Objective To evaluate color Doppler flow imaging（CDFI） and two dimensional ultrasonography in the differential diagnosis of uterine myoma and adenomyoma.Methods Transabdominal and transvaginal color Doppler sonography was performed in 45 cases of uterine myomas and 38 cases of uterine adenomyoma to analyze the characteristics of two dimensional and color Doppler flow with them,50 normal women were chosen for control.Results The age of onset and clinical manifestations of two groups of patients were similar,but dysmenorrhea and uterine tenderness of uterine adenomyoma group were obvious.Characteristic changes were observed in the two dimensional ultrasonography of both the uterine myoma and adenomyoma.CDFI： RI of uterine artery of uterine myoma and uterine adenomyoma were lower than that of control group（P 0.05）.In the group of uterine myoma,color blood flow was often detected in peripheral area of focal uterine lesions,bases of submucous and subserous myoma showed rich blood flow.The blood stream at edge of mass in patients with uterine adenomyoma was sparse or absence,and there was rich blood flow or starlike flicker color blood signal in the interior.The value of RI in the uterine adenomyoma＇s interior was higher than within and at the egde of the uterine myoma.Conclusion Transabdominal and transvaginal color Doppler ultrasonogrphy provide differential diagnosis of uterine myoma from uterine adenomyoma,the diagnostic accuracy of them must be improved with clinical performance.
