周围型小肺癌的CT诊断

摘　　要：目的进一步认识周围型小肺癌的CT征像,提高CT诊断正确率。方法回顾性分析了56例经组织病理证实直径≤2 cm的周围型小肺癌的SCT、HRCT及增强SCT表现。重点对病灶的内部结构、边缘形态、周围组织改变及增强程度进行总结分析。结果 56例小肺癌中,HRCT具有分叶征47例,毛刺征45例,胸膜凹陷征34例,血管束征21例,胸膜侧阴影16例,空泡征23例,细支气管气象征5例,磨玻璃样改变6例,空洞2例,钙化1例。征像出现率明显高于SCT。增强病例中48例CT值增幅>20 Hu。结论 HRCT显示病灶内部结构和边缘形态明显优于SCT。具有3种边缘征像以上的肺部外围型结节应高度怀疑周围型肺癌,增强扫描对鉴别结节的良恶性很有帮助。CT表现与病理分型有一定联系。Objective To understand further the CT signs of small peripheral lung cancer and to improve its diagnostic rate by CT. Methods 56 patients with small peripheral lung cancer（diameter was approximately 2 cm）were examined with spiral CT scanning.All cases were confirmed by pathology.And additional high resolution spiral CT scanning and enhanced CT scanning was done for them.The size,edge signs,internal texture and enhanced degree were analyzed. Results Of 56 cases,CT findings included the lobular sign in shape in 47 cases,burr sign in 45 cases;sign of hollowed pleura in 34 cases;sign of blood vessel bundle in 21 cases;sign of small bubbles in 23 cases;sign of air bronchus in 5 cases.Ground-glass-like changes in 6 cases,cavity in 2 cases,calcification in 1 cases,enhanced degree20 Hu in 48 cases. Conclusion HRCT is superior to spiral CT in demonstrating the characteristic sign of small lung cancer.Peripheral pulmonary nodules which are more than 3 edge signs on CT are highly suspected to be peripheral pulmonary carcinoma.Enhanced CT scanning is very helpful in distinguishing benign and malignant pulmonary nodules.The CT appearances are well correlated with the type of histology and the size of the lung cancer.
