摘 要:用流式细胞术对60 例初诊或未缓解的急性白血病患者进行免疫表型检测。AML患者均表达髓系抗原,部分病例同时伴有淋巴系抗原的表达。ALL患者均表达淋巴系抗原,但28.6% 病例同时表达髓系抗原(CD13或CD33 。)。2 例急性髓系与淋系双表型杂合性白血病同时表达2个以上淋巴和髓系抗原标志。1 例急性T系与B系双克隆杂合性白血病两群白血病细胞分别表达T、B系抗原。1 例形态学检查为AML,但表达全部3 个B淋巴抗原和CD13 ,而诊断为伴髓系抗原表达的急性淋巴细胞性白血病。结果表明,结合免疫表型分析与细胞形态学检验能为急性白血病的诊断提供更可靠和更有价值的分型依据。patients with primary or partial or not remission leukemia were determined with flow cytometry to obtain valid diagnosis.All myeloid leukemia patients expressed myeloid markers,but some cases also expressed lymphoid lineage antigens.Lymphoid antigen expression was found in all acute lymphoblastic leukemia,but 28 6%(10/35)of these patients also expressed myeloid markers CD 13 or CD 33 .Two cases are myeloid and lymphoid hybrid type leukemia that expressed more than 2 markers of both lymphoid and myeloid origins.One case was T and B hybrid type whose leukemic cells divided into two distinct populations expressing T or B antigens simultaneously.One case displayed morphological characteristics of myeloid leukemia,but immunophenotyping results revealed that this was actually acute lymphoblastic leukemia with myeloid antigen expression,since all three B antigens and CD 13 were strongly expressed.Our data indicate that immunophenotyping,combined with morphology can provide more reliable and valuable evidence in the diagnosis of leukemia.
