溃疡性结肠炎患者正常与病变肠段肠道菌群比较

摘　　要：目的比较溃疡性结肠炎患者正常与病变肠段粘膜上菌群分布的差异,以探讨肠道菌群在溃疡性结肠炎发病机制中的作用。方法选取广州地区40例肠道病变仅局限于乙状结肠以下的溃疡性结肠炎患者,用结肠镜分别在乙状结肠以上的正常肠段和乙状结肠以下的病变肠段取得粘膜标本,另选40例健康成人为正常对照,用荧光定量PCR法检测标本中乳酸杆菌、大肠杆菌、双歧杆菌的含量。结果 (1)健康人乙状结肠以上肠段双歧杆菌、乳酸杆菌及大肠杆菌数量对数值分别为8.71±0.44、9.01±0.33、7.01±0.13,乙状结肠以下病变肠段双歧杆菌、乳酸杆菌及大肠杆菌数量为9.21±0.24、9.02±0.18、7.12±0.21,均无明显差异。(2)溃疡性结肠炎患者正常肠段双歧杆菌、乳酸杆菌及大肠杆菌数量分别为8.92±0.37、9.15±0.17、7.22±0.24,与健康人无明显差异,病变肠段双歧杆菌、乳酸杆菌及大肠杆菌数量分别为6.52±0.76、7.49±0.31、9.67±0.22,双歧杆菌、乳酸杆菌均较健康人为低,大肠杆菌均较健康人为高。结论溃疡性结肠炎患者病变肠段存在肠道菌群失调。Objective To compare the distribution of intestine mucosa flora of normal and abnormal parts in patients with ulcerative colitis（UC） and explore the roles of micro ecosystem in pathogenesis of UC.Methods 40 cases of UC patients with abnormal lesion limited to sigmoid colon in Guangzhou area were chosen to receive colonoscopy to obtain mucosa specimens of normal and abnormal parts.And another 40 healthy adults were chosen as normal control,of which the Bifidobacterium,lactic acid bacteria,and Escherichia coli were determined by fluorescence quantitative PCR.Results 1.Quantitations of mucosa Bifidobacterium and lactic acid bacteria,Escherichia coli over the sigmoid colon bowel in healthy adults were 8.71±0.44,9.01±0.33,7.01±0.13,and those following the sigmoid colon bowel were 9.21±0.24,9.02±0.18,7.12±0.21,there were no significant differences between over and following the sigmoid colon.2.Quantitations of mucosa Bifidobacterium,lactic acid bacteria,and Escherichia coli in normal parts of UC patients were 8.92±0.37,9.15±0.17,7.22±0.24,and those in abnormal parts were 6.52±0.76,7.49±0.31,9.67±0.22,there were no significant differences between normal parts of UC patients and healthy adults.Bifidobacterium and lactic acid bacteria in abnormal parts were lower than those in normal parts of UC patients and healthy adults,while Escherichia coli was higher than that in normal parts of UC patients and healthy adults.Conclusion Alteration of intestinal flora existed in abnormal parts of ulcerative colitis bowel.
