晚期肺腺癌^(18)F-FDG摄取与化疗疗效及生存期的相关研究

摘　　要：目的探讨晚期肺腺癌18F-FDG摄取与化疗疗效及生存期的相关关系。方法 37例晚期肺腺癌患者行SPECT检查,比较不同性别、肿瘤原发病灶的大小及组织分化程度对18 F-FDG摄取(SUV)的差异,并进一步探讨原发灶FDG摄取和化疗疗效及生存期之间的关系。结果性别对晚期肺腺癌的18F-FDG摄取有一定的影响,但无统计学差异;肿瘤原发病灶<3cm组的SUV小于原发病灶≥3cm组,近期疗效及生存期明显好于肿瘤原发病灶≥3cm组,差异有显著性(P<0.05);肿瘤组织高分化组SUV小于中分化及低+未分化组(P<0.05),高分化组近期疗效低于中分化及低分化+未分化组(P<0.05);高分化组生存期好于中分化组但无统计学差异(P>0.05),与低+未分化组差异有显著性(P<0.05),结论晚期肺腺癌原发病灶对18 F-FDG摄取值(SUV)随肿瘤原发灶增大及恶性成度增高而增加;化疗疗效随肿瘤对18F-FDG摄取(SUV)值增大而降低。生存期随肿瘤对18F-FDG摄取增加而降低。Objective To investigate the influence of the 18-fluorodeoxyglucose （18F-FDG） uptake on the chemotherapy effectiveness and life span in patients with advanced adenocacinoma of lung. Methods Totally 37 patients with advanced adenocacinoma of lung were enrolled and underwent 18F-FDG -SPECT imaging. The influences of gender, size of tumor, and histological differentiation on 18F-FDG uptake in patients with advanced adeuocacinoma of lung were assessed. Results Gender had a few influence on 18F-FDG uptake value （T/N）, but without statistical significance. The T/N value significanthy decreased in group with size of primary lesions less 3cm than the group with the size of primary lesions larger than 3cm （P 〈 0. 05 ） , The chemotherapy effectiveness and life span were increased significantly in group with the size of primary lesions less than 3cm （P 〈 0. 05） ; the T/N value and the chemotherapy effect was significantly lower in group with well-differentiated tumors than in groups with moderately 1 poorly differentiated tumors （ P 〈 0. 05 ）. The life span was significantly longer in group with well-differentiated tumors than in groups with poorly differentiated tumors （P 〈 0. 05）. Conclusion The 18F-FDG uptake is positivehy correlated with the size of primary lesions and malignancies in advanced adenoeacinoma of lung.
