胰腺实性假乳头状瘤的影像学诊断

摘　　要：目的探讨胰腺实性假乳头状瘤的CT和MRI表现。方法回顾性分析经手术病理证实的16例胰腺实性假乳头状瘤的影像资料。其中,CT扫描10例,MR扫描2例,CT和MR同时扫描者4例。结果 15例为女性(16岁及55岁),1例为男性(36岁)。3例肿瘤以囊性成分为主,8例实、囊成分比例相仿;5例以实性成分为主;全部16例SPTP均见到由包膜、肿瘤实质、囊性区3种结构构成的不均匀密度影;6例有钙化。CT增强扫描门静脉期肿瘤强化略高于动脉期,但其强化程度均低于正常胰腺组织。MRI 6例在T1WI、T2WI上呈混杂信号,实性成分在T1WI呈中低信号,T2WI呈中高信号,增强扫描呈不均性的中高强化。2例有出血。结论胰腺实性假乳头状瘤的CT、MRI表现有一定特征性,Objective To study the CT and MRI features of solid-pseudo papillary tumors of pancreas（SPTP）.Methods Imaging findings of 8 patients with histopathologically proven SPTP were retrospectively analyzed.Of 16cases,CT was performed in 10 patients,and MRI in 3 patients,CT and MR was 3 patients.Results 15 patients were female（age 16 years and 55 years）,1 patients is man（age 36 years）.There tumors were predominantly composed of cystic portions,eight tumor was composed of similar proportions of solid and cystic portions,and five tumors were mainly composed of solid portions.16 cases of density with SPTP were not even.All of the mass showed density shadow of tumor capsule,tumor epithelial cells.Cystic regions.Calcification was seen in 6 cases on CT.Enhancement of solid portions was slight higher in portal venous phase than arterial phase on post-contrast CT,but was lower in both phase than normal pancreas.MR six showed mixed signal intensities on T1WI and T2WI,and solid portions of mass showed moderate hypo intensity on T1WI,and moderate hyper intensity on T2WI,and heterogeneous enhancement enhanced T1WI MRI in 6 cases.Bleeding was seen in 2 cases on.Conclusion There are some characteristics in CT and MRI manifestation of SPTP.
