肠内外联合营养支持对急性脑卒中患者临床价值的回顾性调查研究

摘　　要：目的探讨肠内外联合营养支持对急性脑卒中患者的临床价值。方法采用回顾性调查研究方法,分析了重庆医科大学近年来收治的急性脑卒中患者774例,其中肠内外联合营养支持(EN+PN组)256例,肠内营养支持(EN组)287例,肠外营养支持(PN组)231例。比较三组患者的营养状况、肝功能、免疫状况、并发症、神经功能恢复情况及对临床疗效的影响。结果 第21天时,EN+PN组各项营养指标,IgA、IgG含量,CD4细胞和CD4/CD8比值,感染并发症发生率,NIHSS评分,总有效率等指标明显优于EN组和PN组,且EN组明显优于PN组。结论肠内外联合营养支持对急性脑卒中患者的临床价值高于肠内营养和肠外营养,且肠内营养的临床价值高于肠外营养。Objective To explore the clinical value of combined enteral and parenteral nutritional support in patients with acute stroke.Methods We reviewed 774 cases of acute stroke who were recently enrolled in Chongqing Medical University during this retrospective study.These cases included 256 patients with combined enteral and parenteral nutritional support（EN＋ PN group）,287 patients with enteral nutritional support（EN group）,and 231 patients with parenteral nutritional support（PN group）.The nutritional status,hepatic function,immunologic status,complications,neurological functional rehabilitation and therapeutic effects were compared between three groups.Results Compared to EN group and PN group,EN＋ PN group showed better results in nutritional status,immunologic status（IgA,IgG,CD4 and CD4/CD8）,the incidence of infective complication,NIHSS scores and the total effective rate.EN group also showed better results than PN group in these indexes.Conclusion Combined enteral and parenteral nutritional support in patients with acute stroke is of better clinical value than enteral nutritional support and parenteral nutritional support,so is the enteral nutritional support.
