非小细胞肺癌手术前后相关肿瘤标志物变化及与预后的相关性分析

摘　　要：目的观察非小细胞肺癌患者手术前后血清CEA、CA125、CYFRA21-1的变化及与手术治疗效果的关系。方法手术前、后常规采集确诊为非小细胞肺癌(NSCLC)的57例患者血清标本,采用电化学发光免疫分析方法检测血清中CEA、CA125、CYFRA21-1的表达情况,评估其变化,并随访2年。结果 57例NSCLC患者中,术前血清肿瘤标志物增高者37例(64.9%),血清CEA、CA125、CYFRA21-1增高率分别为40.4%、33.3%及45.6%;Ⅰa期NSCLC分别为0%0、%和0%,Ⅰb期NSCLC分别为25%、12.5%和12.5%;Ⅱa期NSCLC分别为35.7%、28.6%和57.1%,Ⅱb期NSCLC分别为55.6%、44.4%和55.6%;Ⅲa期NSCLC分别为55.0%、50.0%和60.0%。手术后,NSCLC血清CEA、CA125、CYFRA21-1降低或降至正常值分别为78.3%、57.9%、76.9%。术后NSCLC血清肿瘤指标较术前增高的患者2年生存率为63.2%,术前增高术后降低或降至正常的患者术后2年生存率为76.3%。结论手术前后血清肿瘤标志物变化与手术治疗效果有关;术后肿瘤标志物明显降低者,患者预后较好。Objective To explore the correlation between serum tumor marker levels of CEA,NSE and CYFRA21-1 with therapeutic response of patients with lung cancer.Methods Serum specimens from 57 lung cancer patients were examined according to electrical chemiluminescent immunoassay before and after operation and the patients were followed up for 2 years.Results In 57 lung cancer patients,the overall rate of positive tumor makers was 64.9%,with those of NSCLC,respectively positive percentage of CEA,CA125 and CYFRA21-1 were 40.4%、33.3% and 45.6%,Ⅰa of NSCLC was 0%、0% and 0%,Ⅱb of NSCLC was 25%、12.5% and 12.5%;Ⅱa of NSCLC was 35.7%、28.6% and 57.1%,Ⅱb of NSCLC was 55.6%、44.4% and 55.6%,Ⅲa of NSCLC was 55.0%、50.0% and 60.0%;After operation,those of NSCLC,respectively positive percentage of reducing of CEA,CA125 and CYFRA21-1 were 78.3%、57.9%、76.9%.The 2-year survival of NSCLC patients whose tumor markers becoming higher after operation were 63.2%,and those of NSCLC patients with normal or reductive tumor markers after operation were 76.3%.Conclusion These serum levels of tumor markers were related to operation.The prognosis was poor and the survival time was shorter in patients with higher serum tumor markers.
