摘 要:目的:探讨导流杂交基因芯片技术(HybriMax)联合薄层液基细胞学(TCT)在宫颈癌筛查临床应用。方法:2009年1月~2010年3月1 500例患者的宫颈脱落细胞为标本来源,采用薄层液基细胞学技术进行细胞学检查,同时采用导流杂交基因芯片技术进行高危型人乳头瘤病毒分型检测,TCT≥ASCUS或HPV阳性的患者进行阴道镜下活组织病理学检查。结果:1 500例受检者TCT结果异常238例,占15.9%,HPV阳性者316例,占21.1%;CINⅡ以上检测敏感率TCT为75.9%~100.0%、HPV为80.0%~100.0%、TCT联合HPV检测为89.8%~100.0%,TCT联合HPV检测明显优于单次HPV检测或者单次TCT的检查。结论:HPV基因分型(HybriMax)与TCT联合检测时可提高宫颈癌前病变的检出率,是一种高效、便捷的宫颈癌筛查方法。Objective:To explore the clinical application of HybriMax combined with thin-layer liquid-based cytology technology(TCT) in screening of cervical cancer. Methods:The cervical exfoliative cells of 1 500 patients were collected from January 2009 to March 2010,TCT was used for cytological detection,at the same time,HybriMax was used for high risk human papillomavirus(HPV) detection,the patients with TCT≥ASCUS or positive HPV received biopsy and pathological examination under colposcope. Results:Among 1 500 patients,238 patients were found with abnormal TCT results,accounting for 15.9%,316 patients were found with positive HPV,accounting for 21.1%;for the patients with lesions above CINⅡ,the sensitivities of TCT,HPV detection and TCT combined with HPV detection were 75.9%100.0%,80.0%100.0% and 89.8%100.0%,respectively,the sensitivity of TCT combined with HPV detection was superior to those of single HPV detection and single TCT. Conclusion:HybriMax combined with TCT can increase the detection rate of cervical precancerous lesions,which is an effective and convenient screening method of cervical cancer
