摘 要:目的:研究盐酸曲美他嗪(Trimetazidine,TMZ)对急性心肌梗死大鼠血浆游离脂肪酸(Free fatty acid,FFA)浓度和左心室梗死周边区心室肌瞬时外向钾电流(Transient outward current,Ito)通道α亚单位(Kv4.2、Kv4.3和Kv1.4)表达变化的影响。方法:通过结扎大鼠左冠状动脉前降支建立心梗模型,将存活的26只大鼠按随机数字表分为急性心梗组和TMZ组,每组13只,同时设立假手术组,10只。4周后取左室梗死周边区心肌组织,分别用Real-time PCR和Western blot检测Kv4.2、Kv4.3、Kv1.4mRNA和Kv4.2、Kv4.3、Kv1.4蛋白质表达,同时取全血用比色法测血浆FFA浓度。结果:急性心梗组血浆FFA浓度和Kv1.4基因表达水平高于假手术组(P<0.05),而Kv4.2、Kv4.3基因表达较假手术组降低(P<0.05);TMZ组血浆FFA浓度和Kv1.4基因表达与心梗组相比降低(P<0.05),而Kv4.2、Kv4.3基因表达水平较心梗组升高(P<0.05),接近于假手术组。结论:急性心梗后4周梗死周边区心肌Kv4.2、Kv4.3基因表达降低,Kv1.4基因表达和血浆FFA浓度增高,TMZ可能通过降低心梗后血浆FFA浓度,逆转其Ito通道α亚单位表达的改变。Objective:To investigate the effects of trimetazidine on serum FFA and the expressions of transient outward current(Ito) channel a subunits-Kv1.4,Kv4.2 and Kv4.3 of right ventricule after acute myocardial infarction(AMI) in rats.Methods:Sprague-Dawley rats AMI model was induced by ligating the left anterior descending coronary artery,and then 26 AMI rats were randomly and equally divided into AMI group and trimetazidine group(60 mg/(kg·d) for 4 weeks by gavage).Another 10 rats served as sham operation group.Colorimetry was used to measure the serum FFA.Real-time PCR and Western blotting were used to measure the mRNA and protein expressions of Kv1.4,Kv4.2 and Kv4.3 in the heart muscles of each group.Results:The mRNA and protein expressions of Kv1.4,and the content of FFA were up-regulated in AMI group compared with sham operation group.The mRNA and protein expressions of Kv4.2 and Kv4.3 were down-regulated in AMI group compared with sham operation group.The expression of Kv1.4 and the content of FFA in trimetazidine group were decreased compared with AMI group.The mRNA and bprotein expressions of Kv4.2 and Kv4.3 were increased compared with AMI group.Conclusion:The expression of Kv1.4 and the content of FFA were up-regulated,and Kv4.2,Kv4.3 were down-regulated in myocardial infarction rats.Trimetazidine may reverse the changes of expression of Ito channel a subunit.
