摘  要:目的构建一个新型后路腰椎融合模型,并与腰椎后外侧融合进行对比,评价其可行性。方法将32只杂种狗,随机分为两组:实验组于L5~6节段行新型后路腰椎融合术;对照组于L5-6节段行腰椎后外侧横突间融合术。于术后8周和16周处死。取出后的标本进行CT三维重建和免疫组化检测。结果 CT三维重建显示实验组融合率高于对照组,16周时差异有统计学意义(P〈0.05);实验组骨痂的灰度值在8周(P〈0.05)和16周(P〈0.01)均高于对照组。实验组BMP-2的表达在两个时间点均高于对照组(P〈0.01)。而实验组VEGF的表达在8周时高于对照组(P〈0.01),16周时低于对照组(P〉0.05)。结论新型后路腰椎融合模型的构建具有可行性,该新型植骨方式融合效果优于腰椎后外侧横突间融合。Objective To construct a novel posterior lumbar spine fusion model and to compare with the conventional posterolateral lumbar intertransverse process fusion.Methods Thirty-two mongrel dogs were randomly assigned to two groups to undergo either novel posterior spinal fusion(experimental group) or posterolateral intertransverse process fusion(control group) at L5-6 segment.The animals were sacrificed at 8 weeks or 16 weeks after surgery.The lumbar spines were evaluated by three-dimensional reconstruction of CT and immunohistochemistry.Results From three-dimensional reconstruction of CT showed that the fusion rates of experimental groups were higher than control group,and there was statistical significance at 16 weeks(P0.05).The gray scales of osteotylus were higher in experimental groups than control groups,and there were statistical significance both at 8 weeks(P0.05) and 16 weeks(P0.01).The expressions of BMP-2 were higher in experimental groups than control groups at both time points(P0.01).Whereas,the expression of VEGF was higher in the treatment group than controls at 8 weeks(P0.01) and lower at 16 weeks(P0.05).Conclusion There is feasibility for construction of a novel posterior lumbar spine fusion model.And the rate and quality of fusion in the experimental spinal fusion model are higher than the conventional posterolateral lumbar intertransverse process fusion.
