旋转式伽玛刀治疗原发性三叉神经痛68例分析

摘　　要：目的分析和评价伽玛刀治疗原发性三叉神经痛的疗效。方法回顾性分析对本院治疗的68例原发性三叉神经痛病人的病例资料。本组病人治疗时采用MRI定位,以患侧三叉神经根为靶点,选用4 mm准直器治疗,中心剂量为70~90 Gy伽玛刀治疗。结果随访4~100个月,疼痛完全缓解50例,疼痛大部分缓解18例,疼痛部分缓解9例,疼痛无缓解2例,其中3例疼痛完全缓解后复发,2例再次经伽玛刀治疗后疼痛完全缓解。1例发生患侧面部麻木。结论伽玛刀治疗原发性三叉神经痛,创伤小、并发症少、疗效好,复发后再次行伽玛刀治疗疼痛仍可缓解。Objective To evaluate the therapeutic efficacy of gamma knife for the treatment of idiopathic trigeminal neuralgia1.Methods An analysis of 68 cases with idiopathic trigeminal neuralgia treated by gamma knife.In our department in the past 7 years was performed1 In all cases,4 mm isocenter was targeted at the proximal nerve at the root entry zone located by MRI1 The target dose varied from 70 ～90 Gy.Results After a follow-up of 4 ～100 months,complete relief of pain occurred in 52 patients（76.5 %）,almost relieved in 10 patients（14.7 %）,partly relieved in 4 patients（5.9%）,no relief in 2 patients（2.94 %）.Repeated radiosurgery was performed in 3 patients with recurrent pain.After the second radiosurgical procedure,complete relief of pain was achiveved in them.2 patients experienced facial numbness after radiosurgical procedure1.Conclusion Gamma knife radiosurgery is a minimally invasive technique for the treatment of trigeminal neuralgia with few complications1 Repeated radiosurgical procedure may result in relief of pain,but the long-term outcome of gamma knife surgery for trigeminal neuralgia needs to be further studied.
