术中区域性缓释化疗对进展期胃癌的疗效及不良反应观察

摘　　要：目的了解进展期胃癌术中行区域性缓释化疗的方法及临床疗效与不良反应情况。方法将郑州市第二人民医院120例进展期胃癌患者随机分为两组,治疗组60例:行胃癌根治术(D2),术中行区域性缓释化疗药物5-FU缓释剂的应用。对照组60例:术中动脉灌注化疗(IAIC)的应用。治疗后对其两组患者的并发症及药物不良反应进行对比分析,并对所有患者进行随访了解术后复发情况。结果治疗组60例共出现并发症6例(10%),对照组60例共有18例出现并发症(30%)。两组1年生存率比较无显著差异性(P>0.05)。两组2年、3年生存率比较有显著差异性(P<0.05)。结论进展期胃癌术中行区域性缓释化疗与术中动脉灌注化疗其疗效显著,但术中行区域性缓释化疗更为直接,其药物不良反应较少。Objective To understand the advanced gastric cancer chemotherapy intraoperative regional release and clinical effect and adverse reactions.Methods Totally 120 patients with advanced gastric cancer were randomly divided into two groups,treatment group 60 patients： patients with gastric cancer（D2）,intraoperative regional slow-release chemotherapy drugs 5-FU release agent.Control group,60 patients： intraoperative arterial chemotherapy（IAIC） application.Its two groups of patients after treatment,complications and adverse drug reactions were analyzed,and all patients were followed up for recurrence of the situation.Results Of 60 patients complications in 6 cases were 10%,control group 60 patients there were 18 complications account for 30%.Treatment group was significantly better than the control group.CEA follow-up treatment group was not found recurrence,metastasis;control group peritoneal metastasis in 1 case.There were no deaths occurred.Conclusions The intraoperative advanced gastric cancer chemotherapy and surgery in the release of regional arterial chemotherapy has significant effect,but the regional release intraoperative chemotherapy is more direct,less adverse drug reactions.
