摘  要:目的:探讨精神病患者对服用氯氮平适应性的体外实验预测方法。方法:抽取600例受试者静脉血1 ml,注入含3.8%柠檬酸钠抗凝剂0.1 ml的试管内,加入10 g/L氯氮平乙醇溶液100 ul,混匀,37℃温浴2 h,制片,瑞氏染色。用显微镜观察粒细胞的结构变化。结果:在600例受试者中,585例粒细胞完整,结构无变化。15例受试者粒细胞出现细胞核肿胀,细胞膜破损、裸核。1个月后随访,585例氯氮平药物敏感度试验正常者,白细胞数均在正常范围内;15例氯氮平药物敏感度试验异常者,有12例出现白细胞减少,(<3.5×109/L)。结论:通过本实验预测患者对氯氮平药物是否敏感,可早期、方便、有效的预防因服用氯氮平所致粒细胞缺乏的发生。Objective:To investingate the in vitro experimental method of forecasting whether the psychiatric patients are adaptable to taking clozapine.Method:One ml venous blood samples was taken from the 600 patients in need of clozapine,mixed with 0.1 ml 3.8% sodium citrate as anticoagulant,and then added 100 ul ethanol solution containing of clozapine 10 g/L.The sample was shaked well and laid in the warm bath of 37 ℃ for 2 hours,then smears were made by Wright's staining,and the structure of granulocytes could be observed by microscope.Results:Six hundred patients taking clozapine were included in our study.In 585 cases,the granulocytes were normal and unchanged,while in other 15 cases,the membrane of granulocytes was injured,and the nucleus were swelled or naked.Follow-up examinations after 1 month shwoed that the 585 patients were tolerated of clozapine,shoes number of granulocytes were normal.Of the 15 patients,whose granulocytes were injured in the cxperiment,12 patients became leukopenia.Conclusion:By using our experimental method,patients sensitivity of clozapine can be early predicted,and the mortal agranulocytosis caused by clozapine can be predicted early,conveniently and effectively.
