高危型HPV、p16、COX-2在宫颈癌中的表达及临床意义

摘　　要：目的检测高危型人类乳头瘤病毒(HPV16/18)、p16、环氧合酶-2(COX-2)在宫颈癌中的表达情况,并探讨其在宫颈癌发生、发展中的意义。方法收集宫颈活检或手术标本151例,采用原位杂交技术检测HPV16/18表达情况,采用免疫组化技术检测p16、COX-2表达情况。结果正常宫颈、宫颈低级别上皮内瘤变(LSIL)、宫颈高级别上皮内瘤变(HSIL)和CC组织中HPV16/18的阳性率分别为9.09%、42.86%、69.23%和85.48%,p16阳性率分别为4.55%、53.57%、76.92%和88.71%,COX-2阳性率分别为9.09%、46.43%、64.10%和79.03%。随着宫颈癌组织病理学分级的升高,p16阳性率逐渐上升,差异有统计学意义(P=0.019)。COX-2表达与宫颈癌临床分期、组织病理学分级及有、无淋巴结转移有关(P<0.05)。结论高危型HPV、p16及COX-2的高表达在宫颈癌的发生、发展中均起作用。Objective To detect the expression of high-risk human papilloma virus（HPV16/18）, p16 and Cyclooxygenase-2 （COX-2） in cervical carcinoma,and inestigatel the significance of it in invasive cervical carcinoma. Methods 151 cases of submission of cervical biopsy or surgical specimens were collected. The expression of HPV16/18 was detected by hybridization in situ. The expression of P16 and COX-2 was analyzed by immunohistocbemistry. Results Positive expression rates of HPV16/18 were 9.09%,42.86%, 69.23% and 85.48% in the normal,LSIL, HSIL and CC samples, 4. 55%,53.57%,76. 92% and 88.71% for pl 6,9.09 %, 46.43 %, 64.10 % and 79.03 % for COX-2, respectively. With the increase of tumor differentiation, positive expression rates of p16 were increased gradually（P= 0.019）. COX-2 expression was found to be correlated with clinical stages, histopathologi cal types and metastasis in lymph node（P〈0.0 5）. Conclusion Expressions of HPV16/18,pl6 and COX-2 are probably involve in the process of cervical carcinoma. Expressions of high-risk HPV, p16 and COX-2 in cervical carcinoma can be detected,and might provid clinical guidance on the early diagriosis of cervical cancer, evaluation of surgery and chemotherapy,and assessment of prognosis.
