胃癌原发灶超声征象多元因素分析与淋巴结转移相关性的研究

摘　　要：目的通过分析胃癌原发灶的声像图表现,探讨胃癌原发灶声像图特征与淋巴结转移的相关性。方法回顾性分析155例胃癌患者的声像图表现,采用单因素分析、多因素Logistic回归分析的方法,分别检验胃癌原发灶的最大径、部位、血流丰富程度、浸润深度、Borrmann分型与淋巴结转移的相关性。结果依据单因素分析,胃癌癌肿浸润深度较深,血流较丰富,肿瘤直径>5cm者,对应淋巴结转移率较高(P<0.05);依据多因素分析,胃癌癌肿最大径、浸润深度及血流特征与淋巴结转移明显相关;结论依据胃癌原发灶声像图特征,术前预评估淋巴节转移的可能性具有重要临床意义。Objective To study the relation between sonographic characters in patients with gastric carcinoma and lymph node metastases.Methods It was retrospectively analyzed sonographies of 155 patients with gastric carcinoma,sonographic characters of multiple factors in patients with gastric carcinoma was studied,including blood supply in primary tumor and its length,TNM classification, Borrmann analysis and tumor localizations.Univariate,Multivariate logistic regression analyses were conducted to determine the value of each sonographic feature.Results Every ultrasonographic feature assessed were correlated with lymph node metastases by Univariate analysis（P0.01）.It was suggested that length of primary tumor,blood supply and TNM classification were the sonographic features indicating lymph node metastases,especially TNM classification of primary tumor by multivariate analysis.Conclusion Lymph node metastases in gastric cancer is mainly correlated with TNM classification,length of primary tumor,blood supply,microscopic type,et.Based on sonographic characters of gastric cancer to pre-assessment of preoperative lymph node metastases have great value in clinical application.
