摘  要:目的:探讨CA 125、CA 19-9、血清铁蛋白等多种肿瘤标志物在非霍奇金淋巴瘤(NHL)中的表达情况及其临床价值。方法:检测172例NHL患者的血清肿瘤标志物(包括血清铁蛋白、CA 125、CA 19-9、CEA、AFP、CA153、PSA、β-HCG)的表达水平,判断CA 125的表达与NHL患者临床特征及其预后的相关性。结果:NHL患者血清铁蛋白、CA 125、CA 19-9阳性率分别为38.4%、32.6%、9.88%,明显高于其他特异性肿瘤标志物。其中,CA 125升高与B症状,腹腔、纵膈淋巴结肿大,临床分期,IPI预后指数,LDH,β-MG,淋巴细胞绝对数减少,贫血等临床特征相关,且CA 125表达水平的高低与淋巴瘤患者的恶性程度和治疗效果密切相关。结论:血清肿瘤标志物的检测特别是CA 125的检测,对NHL的临床分期、预后和疗效判断方面具有重要参考价值。Objective:To investigate the clinical significance of serum tumor markers in the diagnosis and prognostic judgement of non-Hodgkin lymphoma(NHL).Method:The serum levels of tumor markers(including SF,CA 125,CA 19-9,CEA,AFP,CA153,PSA and β-HCG) were detected by electrochemiluminescence method in 172 patients with NHL.The relationship between CA 125 and clinical characteristics was explored.The relationships between CA 125 and prognosis were studied as well.Result:The positive rates of SF,CA 125 and CA 19-9 in patients with NHL were 38.4%,32.6% and 9.88% respectively,which were obviously higher than those of other serum tumor markerse.CA 125 levels correlated with the presence of B symptoms,abdominal or mediastinal involvement,advanced clinical stage,high IPI index,high LDH level,high β2-microglobulin level,low absolute lymphocyte count(ALC) and anemia.We also found that patients with elevated serum CA 125 levels had significantly more adverse prognostic factors at diagnosis,lower CR rate,higher relapse rate and worse survival.Conclusion:The detection of serum tumor markers,especially serum CA 125 measurement,is of value in diagnosis,clinical stage and prognosis assessment in patients with NHL.
