摘  要:目的 分析脑膜瘤瘤周水肿与肿瘤大小的相关性.方法 收集手术及病理证实的96例首发脑膜瘤的头颅磁共振成像资料并计算瘤周水肿指数.结果 小、中、大脑膜瘤(分别为<2 cm,2~5 cm及>5 cm直径)瘤周水肿的发生率分别为10.0%(1/10)、48.1%(25/32)、88.2%(30/34),随肿瘤增大瘤周水肿呈逐渐增高趋势.所有脑膜瘤组水肿指数与脑膜瘤体积大小无相关性(r=-0.141,P>0.05),但有瘤周水肿脑膜瘤组中水肿指数和脑膜瘤体积大小呈负性相关性 (r=-0.279,P<0.05).当脑膜瘤体积(V)≤20 cm3、20~40 cm3、40~60 cm3及>60 cm3时,瘤周水肿发生率分别为33.3%(13/39)、52.3%(11/21)、71.4%(10/14)、100.0%(22/22).结论 脑膜瘤体积与瘤周水肿发生率有关,但与瘤周水肿的严重程度无关 脑膜瘤体积越大,瘤周水肿发生率越高.Objective To analyse the correlation between the peritumoral edema and the size of meniningiomas. Methods 96 cases of meningiomas confirmed by pathology underwent MR imaging preoperation. The index of peritumoral edema were calculated. Results The incidences of peritumoral edema in small (〈2 cm in diameter) ,medium (2-5 cm in diameter)and large(〉5 cm in diameter)meningiomas were 10.0% (1/10),48. 1% (25/32),88.2%(30/34),respectively. It showed that the incidence was gradually increased with the tumors increased in size. There was no correlation between the edema index of al1 meningiomas and the volume of meningiomas (r=-0.141, P〉0.05 ), but in the group of meningiomas with edema,there was negative correlation between the edema index and volume of meningioma (r=-0.279 , P〈0.05 ) . When the volumes of meningioma(V)were≤20 cm3 ,20-40 cm3 ,40-60 cm3 and 〉60 cm3 ,the incidences of brain edema were 33.3%(13/39),52.3%(11/21),71.4%(10/14) and 100.0%(22/22),respectively. Conclusion There is correlation between the volume of meningiomas and the incidence of peritumoral brain edema,but not with the severity of brain edema, the larger in size of the meningioma, the higher of the incidence of peritumoral brain edema.
