摘　　要：目的探讨经皮肾镜超声/气压弹道碎石清石术（PCNL）治疗上尿路结石的疗效。方法回顾性分析采用B超引导PCNL治疗82例上尿路结石患者的临床资料。结果 82例均一期成功建立经皮肾通道,5例行一期双通道取石,1例行一期3通道取石,手术时间30~130 min。术后3 d内体温38.5℃以上10例（12%）;无血气胸、腹腔脏器损伤等严重并发症发生。术后1周复查,73例（89%）取尽结石,9例患者有残石行二期PCNL或体外冲击波碎石（ESWL）处理。结论 B超引导PCNL治疗上尿路结石安全、可靠。Objective To evaluate the feasibility and safety of percutaneous nephrolithotripsy（PCNL） with pneumatic and ultrasonic power under ultrasound guidance in the treatment of upper urinary calculi.Methods Data of 82 cases with upper urinary calculi underwent PCNL were retrospectively analyzed.Results The percutaneous renal access guided by B-type ultrasound was successfully established in all patients,of whom 5 patients were via two tracts and 1 patient via three tracts.Operation time ranged from 30 to 130 minutes.No severe complications occurred during nephrolithtripsy.Fever over 38.5℃ in three days after operation was seen in 10 cases.Stones were cleared in 73 cases （89%）,residual stone fragment was found in 9 cases,who received second PCNL or extracorporal shock wave lithotripsy.Conclution Percutaneous nephrolithotripsy with pneumatic and ultrasonic power under B-type ultrasound guidance for treatment of upper urinary calculi is effective and safe.
