摘  要:目的:观察初治白血病患者神经生长因子(NGF)及其低亲和性受体CD271的表达水平变化及其临床意义。方法:运用酶标记免疫吸附分析(ELISA)测定40例急/慢性白血病患者(病例组)血清NGF水平以及流式细胞术(FCM)测定白血病细胞群CD271阳性表达率,同时选择35例健康体检者为对照组。结果:病例组患者血清NGF水平较对照组有明显升高(P<0.01),但急性髓细胞性白血病(AML)、急性淋巴细胞性白血病(ALL)和慢性粒细胞性白血病(CML)患者间比较差异无统计学意义(P>0.05)。病例组患者白血病细胞群表面CD271表达水平与对照组比较明显降低,差异有统计学意义(P<0.01),但不同免疫分型间比较差异无统计学意义(P>0.05)。经两个化疗周期后,总体缓解率为60.00%(21/35),未缓解组NGF和CD271表达水平与对照组比较差异有统计学意义(P<0.01),缓解组NGF和CD271表达水平与对照组比较差异均无统计学意义(P>0.05)。结论:白血病患者血清NGF高表达以及白血病细胞CD271低表达可能参与白血病发生发展,且可能是判断近期疗效的有效指标。Objective To investigate the expressions of nerve growth factor(NGF) and the low-affinity receptor CD271in primary dignosied leukemia patients and analyze their clinical significance.Methods The expressions of NGF in plasma and CD271 on leukemia cells were detected by ELISA and flow cytometry repectively in 40 acute/chronic leukemia cases(disease group),and 35 healthy subjects were selected as control group.Results The level of NGF in disease group was significantly higher than that in control group(P0.01),but no statistical significance were showed among acute myelocytic leukemia(AML),acute lymphocytic leukemia(ALL) and chronic myelogenous leukemia(CML) groups(P0.05).The CD271 expression on leukemia cells in disease group was obviously lower than that in control group(P0.01),but it's expression was comparable in the 3 groups(P0.05).The total remission rate was 60.00%(21/35) after 2 cycles of chemotherapy.The levels of NGF and CD271 in non-remission group were significantly higher and lower than those in control group individually(P0.01),but remission group was the absolute contrary(P0.05).Conclusion The NGF high expression and CD271 low expression in leukemia patients may play a part in the development of leukemia and may be a useful index to evaluate their short term effects.
