f/t-PSA比值对t-PSA在正常范围内的Pca患者早期诊断的临床意义

摘　　要：目的：研究前列腺特异性抗原游离与总量（f/t-PSA）比值对t-PSA在正常范围内前列腺癌（Pca）早期诊断的价值。方法：采用电化学发光免疫分析技术检测150例正常健康男性,106例t-PSA〉4.0μg/L及22例t-PSA〈4.0μg/L的Pca患者血清t-PSA和f-PSA并求出f/t-PSA比值,然后进行统计学分析。结果：正常对照组;t-PSA〉4.0μg/L Pca对照组;t-PSA〈4.0μg/L Pca组的t-PSA、f-PSA及f/t-PSA比值分别为：1.39±1.15、0.45±0.32、0.38±0.16;33.14±21.97、4.34±3.27、0.16±0.09;2.72±1.23、0.44±0.31、0.17±0.10,正常对照组与t-PSA〈4.0μg/L Pca组的t-PSA比较和f/t-PSA比较均有显著性差异（P〈0.001）,而f-PSA之间比较无显著性差异（P〉0.05）。t-PSA〉4.0μg/L Pca对照组与t-PSA〈4.0μg/L Pca组的t-PSA比较和f-PSA比较均有显著性差异（P〈0.001）,而f/t-PSA比较无显著性差异（P〉0.05）。将f/t-PSA比值分为〈0.10、0.11~0.15、0.16~0.20、0.21~0.25、0.26~0.30以及〉0.31以上六个截点,显示在0.20截点以内t-PSA〈4.0μg/L Pca患者占有率为77.27%,t-PSA〉4.0μg/L Pca患者占有率为74.53,两组之间比较无显著性差异（P〉0.05）。结论：f/t-PSA比值的检测不仅对血清t-PSA〉4.0μg/L的Pca患者有良好的诊断价值,而对t-PSA〈4μg/L的Pca患者同样具有良好的诊断价值,这对t-PSA〈4.0μg/L的Pca患者做到早期诊断显得尤为重要。Objective To investigate the value of f/t-PSA ratio on early detection for prostate cancer（Pca） patients with normal range of prostate specific antigens（PSA）.Methods ①The serum free PSA and total PSA in 150 normal male volunteers（normal control）,and 128 cases with Pca（106 cases with PSA4.0μg/L,22 cases with PSA4.0μg/L） were measured by electrochemiluminescence immunoassay.The ratios of f/t-PSA ratio were therefore obtained and analyzed respectively.Results The serum concentrations of t-PSA,f-PSA and f/t-PSA ratio were 1.39±1.15ng/ml,0.45±0.32ng/ml,0.38±0.16 in cases of normal control group respectively,33.14±21.97ng/ml,4.34±3.27ng/ml and 0.16±0.09ng/ml in 106 cases with PSA4.0μg/L,2.72±1.23ng/ml,0.43±0.31ng/ml and 0.17±0.10ng/ml in 22 cases with PSA4.0μg/L respectively.Significant difference of t-PSA,f/t-PSA ratio was observed between the normal control group and 22 Pca cases with 4.0μg/L（P0.001）.Compared with normal control group,significant difference of t-PSA,f-PSA was found in the group of 106 Pca cases with 4.0μg/L.The cut off of f/t-PSA ratios were ranked as 0.10,0.11~0.15,0.16~0.20,0.21~0.25,0.26~0.30,0.31.The 77.27% and 74.53 prevalence of Pca were observed in patients with PSA4.0μg/L,PSA4.0μg/L individually when cutoff of f/t-PSA ratio was less than 0.20（P0.05）.Conclusion The analysis of f/t-PSA ratio is of great help for early detection of prostate cancer for patients with serum t-PSA4.0μg/L and as low as 4.0μg/L.
