新疆维吾尔族与汉族患者下颌第二磨牙C形根管的临床对比研究

摘　　要：目的对比研究新疆维吾尔族与汉族患者下颌第二磨牙C形根管的发现率及其根管系统的解剖形态,为临床诊断和治疗给予指导。方法门诊随机接诊需行根管治疗的维吾尔族与汉族患者下颌第二磨牙各100例。分别拍摄术前X线片后开髓清理髓室,手术显微镜下观察并记录根管口形态,比较2组C形根管的发现率。并对确诊为C形根管系统的患牙拍初尖锉片(或试尖片)进行分析,评价2组C形根管系统的解剖形态。结果 200例中通过术前X线片和手术显微镜确诊为C形根管系统共49例,维吾尔族14例,汉族35例。通过手术显微镜诊断的41例C形根管,30例为C1型,7例为C2型,4例为C3型。其中维吾尔族C1型9例,C2型2例,C3型1例;汉族C1型21例,C2型5例,C3型3例。49例C形根管的初尖(主尖)片上,23例为Ⅰ型,18例表现为Ⅱ型,8例表现为Ⅲ型。其中维吾尔族Ⅰ型6例,Ⅱ型5例,Ⅲ型3例;汉族Ⅰ型17例,Ⅱ型13例,Ⅲ型5例。结论汉族患者下颌第二磨牙C形根管发现率高于维吾尔族,维、汉族患者下颌第二磨牙在C形根管口形态都以C1形为主;维、汉族患者下颌第二磨牙C型根管系统都以Ⅰ型为主。Objective To compare the incidence of the C-shaped mandibular second molars and the anatomical features of C-shaped canal system in Uygur and Han ethnic group.Methods A total of 200 mandibulars second molars considered as needing root canal therapy were included in this study,in which the Uygur and the Han ethnic group are 100 respectively.After the diagnosis of C-shaped canal System which was based on the configuration of canal orifice observed under the operating microscope in conjunction with the radiographic canal morphology of the preoperative radiographs,the initial radiographs of the verified C-shaped canal systems were classified according to the radiographic features of C-shaped canal system.The incidence of the C-shaped canal of the mandibular second molars and the anatomical structure of the C-shaped canal system were analyzed.Results Of the 200 mandibular second molars,49 cases were diagnosed as having C-shaped canal system,14 cases and 35 cases were the uygur and the Han ethnic group respectively.41 cases of C-shaped canals were diagnosed by the microscope,9,2 and 1 of which were C1,C2 and C3 in the Uygur,and 21,5 and 3 in the Han ethnic group.According to the IAF films,6,5 and 3 were classified as radiographic type,Ⅱ and Ⅲ in the Uygur group,and 17,13 and 5 in the Han ethnic group.Conclusion The incidence of the C-shaped mandibular second molars in the Han ethnic group is higher than that in the Uygur;the most common configuration of canal orifice are C1 and the anatomical structure of the C-shaped canal system are typeⅠin the Han ethnic group and the Uygur.
