致心律失常性右室心肌病的临床特点和心电图分析

摘　　要：目的:探讨致心律失常性右室心肌病(ARVC)患者临床特点和心电图分析。方法:对20例ARVC患者的临床资料进行回顾性分析,分析ARVC患者临床表现及心电图表现。结果:20例ARVC患者中表现为心律失常者11例(55%),右心衰竭者8例(40%),无症状者1例(5%)。心电图有9例(45%)在V1～3导出现Epsi-lon波,右侧胸前导联的平均QRS间期(QRS1,V1+V2+V3/3)为(0.12±0.03)s,左侧胸前导联的平均QRS间期(QRS2,V4+V5+V6/3)为(0.10±0.02)s,QRS1/QRS2≥1.2者12例(60%)。结论:ARVC临床表现以心律失常或右心衰竭为主,右胸导联出现Epsilon波和右侧胸前导联QRS延长有助于诊断ARVC。Objective：To investigate the clinical features and electrocardiogram（ECG） in patients with arrhythmogenic right ventricular cardiomyopathy（ARVC）.Method：There were 20 patients with ARVC who were retrospectively analyzed in the study.The clinical characteristic and ECG of ARVC were analyzed.Result：In the 20 patients with ARVC,there were 11 cases（55%） with arrhythmia,8 cases（40%） with right heart failure,1 case（5%） without symptom.There were 9 cases（45%） found Epsilon waves in right pericardial leads（V1~3）.Mean QRS duration in rig ht pericardial leads（QRS1,V1＋V2＋V3/3） was（0.12±0.03）s.Mean QRS duration in left pericardial leads（QRS2,V4＋V5＋V6/3） was（0.10±0.02）s.QRS1/QRS2≥1.2 was present in 12 patients（60%）.Conclusion：Arrhythmia and right heart failure are the main characteristics in patient with ARVC.Epsilon waves and localized right pericardial QRS prolongation in standard ECG may have high value in the clinical diagnosis of ARVC.
