林州市居民食管贲门癌发病登记与人群筛查资料对照分析

摘　　要：目的探讨林州市居民食管癌、贲门癌发病与人群内镜筛查的不一致性的可能原因。方法应用林州市2006年居民食管癌和贲门癌发病登记资料与食管癌早诊早治示范基地目标人群的内镜筛查资料进行对照。结果 2006年林州市居民共发生食管癌和贲门癌1127例,食管癌733例,占65%,其中40～69岁居民发病数占71.76%;贲门癌394例,占35%,40～69岁居民发病数占72.34%。目标人群40～69岁10425例,内镜筛查了6776例,顺应性65%,发现食管癌48例,贲门癌60例。结论人群内镜筛查贲门癌较食管癌多,而居民登记资料食管癌发病率较贲门癌高。这可能与在筛查中,采用了贲门高发位点的常规活检,使得无症状的早期贲门癌症病人得到了超前诊断,而居民登记资料来源于临床治疗,多为有明显症状的中晚期癌患者有关。同时是否提示贲门癌和食管癌的发病机理存在差异,尚需进一步研究。Objective To explore the Forest City residents esophageal and cardiac disease and endoscopic screening for inconsistencies among the possible reasons for the phenomenon. Methods Forest City residents in 2006 the incidence of esophageal and gastric cardia and esophageal cancer registration data model based early diagnosis and early treatment of endoscopic screening of the target population data control. Results there were 1127 cases of esophageal of forest city residents in 2006 and gastric cardia,733 cases of esophageal cancer,accounting for 65% of population aged 40～69 accounted for 71.76% incidence;cardiac 394 cases,accounting for 35%,40～69 years old accounted for 72.34% incidence. 10425 cases of the target population 40-69 years of age,endoscopic screening of the 6776 cases,65% complianced and found 48 cases of esophageal cancer,60 cases of gastric cardia. Conclusion Endoscopic screening for gastric cardia crowd is more than the esophagus,and esophageal cancer registration data for residents of cardiac morbidity is high.This may be related to the screening,the use of high cardiac conventional biopsy sites,making the early asymptomatic patients have been advanced cardiac diagnosis,and registration data from the clinical treatment of residents,mostly obvious symptoms in advanced cancer Were related. At the same time,it prompts the pathogenesis of cardiac and esophageal cancer are different,needs further study.
