四肢软组织静脉型血管瘤的MRI诊断价值

摘　　要：目的:探讨四肢软组织静脉型血管瘤的MRI表现和诊断价值。方法:回顾性分析我院经手术病理证实的14例四肢软组织静脉型血管瘤的MRI表现。结果:14例静脉型血管瘤中,病变位于皮下脂肪层4例,肌层6例,肌间隙1例,累及多层3例。形态呈不规则团块状或迂曲管状,边界在T2WI/STIR均显示清晰。T1WI与肌肉相比呈等信号11例,等高混杂信号3例;T2WI/STIR呈高低混杂信号14例;增强扫描呈明显不均匀强化12例,轻度强化2例。结论:MRI可明确四肢软组织静脉型血管瘤的诊断,并能明确病变的位置、形态和范围,为临床诊断及手术方案的制订提供可靠的依据。Objective To study the manifestations and diagnostic value of MRI for venular hemangioma of soft tissue. Methods Retrospective analysis the MRI findings of 14 cases with venular hemangioma of soft tissue which have been treated by operation and comfirmed by pathology. Results In 14 cases of venular hemangiomas ,there were 4 cases in rarefaction tissue ,6 cases in intramuscula, 1 cases in muscular space and 3 cases in several layers. Morphology of lesions was irregular bulk or weaves tubular. The edge of all cases on T2WI/STIR was clear. 11 cases showed equal signal intensity （equal to muscle） and 3 cases showed heterogeneous signal intensity on T1 - weighted images. 14 cases showed heterogeneous signal intensity on T2 - weighted images with fat suppression. 2 cases showed slight and 12 cases showed markedly inhomogeneous enhancement after intravenous contrast injection. Conclusion The diagnosis of venular hemangioma of soft tissue can be verified and the position, the shape and the extent of lesions can also be showed by MRI. It provides a reliable basis for clinical diagnosis and operative plan.
