检测循环内皮细胞评估晚期食管癌放化疗疗效的价值

摘　　要：目的探讨检测外周血循环内皮细胞（CEC）在晚期食管癌放化疗疗效评估中的价值。方法 66例晚期食管鳞癌患者分为化疗组（21例,用PF方案）、放疗组（18例,剂量60~70 Gy/6周）和放化疗联合组（27例）。用流式细胞术检测外周血CEC。结果晚期食管癌患者CEC基础值较健康者高[（20.84±7.31）个/μl vs.（11.14±3.07）个/μl]（P〈0.01）。治疗前,3组CEC数相仿;治疗后,放化疗联合组CEC较治疗前显著降低[（14.39±5.36）个/μl vs.（20.22±7.01）个/μl]（P〈0.01）,其他两组降低不明显。结论检测治疗前外周血CEC有可能成为预测晚期食管癌患者放化疗疗效的指标。Objective To investigate the significance of detecting circulating endothelial cell（CEC）in evaluating the efficacy of radiochemotherapy in the patients with advanced esophageal cancer.Methods Sixty-six patients with advanced esophageal cancer were divided into groups of chemotherapy（group A,21 cases,PF regimen）,radiotherapy（group B,18 cases,60～70 Gy/6w） and chemoradiotherapy（group C,27 cases）.The CECs were detected with flow cytometry.Results The CECs were higher in the patients with advanced esophageal cancer than those in the healthy people [（20.84±7.31） pieces/μl vs.（11.14±3.07） pieces/μl]（P0.01）.Before treatment,the CECs of three groups were similar.The CECs of group C were lower after treatment than those before [（14.39±5.36） pieces/μl vs.（20.22±7.01） pieces/μl]（P0.01）,which were not significantly decreased in groups of A and B.Conclusion Detection of CECs in periphyral blood could be taken as a marker in evaluating the efficacy of radiochemotherapy in the patients with advanced esophageal cancer.
