中人氟安联合亚砷酸治疗晚期肝癌42例临床分析

摘　　要：目的探讨皮下植入中人氟安（氟尿嘧啶缓释剂,5-fluorouracil sustained-release,5-FU）联合静滴亚砷酸注射液（Arsenous acid）治疗晚期肝癌的疗效及不良反应。方法所有患者均为晚期肝癌和术后或介入术后复发的病例,对照组55例予静滴亚砷酸每日10 mg,连用14 d为一疗程;治疗组42例在对照组用药基础上,加予一次皮下植入中人氟安300500 mg。评价两组的临床疗效、毒副反应、生存质量（KPS评分）、生存期等情况。结果治疗组、对照组的总有效率为61.9%、34.5%,临床获益率为85.7%、65.5%;治疗组、对照组患者平均生存期分别为（1.41±0.32）年、（0.98±0.26）年（P〈0.05）;两组均有部分患者出现轻-中度胃肠道反应,肝功能异常或黄疸加重,骨髓抑制等不良反应,但两组各不良反应发生率差异无统计学意义（P〉0.05）,且经对症治疗后病情缓解。结论皮下植入中人氟安联合静滴亚砷酸治疗晚期肝癌效果显著,延长患者生命,提高生活质量;两药具有协同作用,毒副反应轻,为治疗晚期肝癌提供一种有效的治疗方法。Objective To investigate the safety and side effect of subcutaneous implantation of sustained release of 5-Fluorouracil plus arsenious acid in the treatment of advanced liver cancer.Methods All patients were of advanced liver cancer or recurrence after surgery or interventional cases,55 cases in control group given intravenous arsenic trioxide（arsenic trioxide） 10 mg daily,for a course of 14 days;42 cases in the treatment group received a subcutaneously implanted sustained release of 5-Fluorouracil 300 mg to 500 mg based on the medication as in control group.The clinical evaluation of efficacy,toxicity,quality of life（KPS score）,survival rate and so on were observed.Results The total effective rate of the treatment group and control group were 61.9% and 34.5%,clinical benefit rate were 85.7% and 65.5%,respectively;the average survival time of the treatment group and the control group were 1.41±0.32 years and 0.98±0.26 years（P〈0.05）,respectively;the patients in both groups have some light-moderate gastrointestinal tract reactions,abnormal liver function or increased jaundice,bone marrow suppression,but the adverse reaction rates showed no significant between the two groups（P〉0.05）,and symptomatic remission after treatment.Conclusion The effect of the subcutaneous implantation of sustained release of 5-Fluorouracil plus arsenious acid for advanced liver cancer was significantly,it could prolong the life of patients and improve their quality of life;Both drugs,with synergistic and light toxicity,can provide an effective treatment of advanced liver cancer treatment,but long-term effect needs further observation
