PAPP-A与动脉粥样硬化性缺血性脑血管病关系

摘　　要：目的探讨动脉粥样硬化性缺血性脑血管病病人血清妊娠相关蛋白-A（PAPP-A）水平与C-反应蛋白（CRP）和基质金属蛋白酶-9（MMP-9）的关系。方法将126例缺血性脑血管病病人分为脑梗死急性期组50例,脑梗死非急性期组40例,短暂性脑缺血发作（TIA）组36例,另选50例健康人作为对照组。测定各组血清PAPP-A、MMP-9和CRP水平。结果脑梗死急性期组和TIA组病人血清PAPP-A水平明显高于脑梗死非急性期组和对照组（F=39.43,q=9.72~11.87,P〈0.01）。脑梗死非急性期组血清PAPP-A水平略高于对照组,但差异无统计学意义。脑梗死急性期组和TIA组血清PAPP-A水平与CRP、MMP-9水平均呈正相关（r=0.424~0.472,P〈0.01）。结论血清PAPP-A水平升高与炎症机制相关,可能提示缺血性脑血管病病人头颈部动脉粥样硬化斑块的不稳定性。Objective To discuss the association of serum level of pregnancy-associated plasma protein-A（PAPP-A） in atherosclerotic ischemic cerebrovascular disease（AICVD） with C-reactive protein（CRP） and matrix metalloproteinase-9（MMP-9）.Methods This study consisted of 126 patients with AICVD,who were divided into acute cerebral infarction group（group 1,50 cases）;non-acute cerebral infarction（group 2,40 cases）;transient ischemic attack（group 3,36 cases）.Fifty healthy subjects served as controls.Serum levels of PAPP-A,CRP and MMP-9 were measured in all subjects of the four groups.Results Serum level of PAPP-A in groups 1 and 3 was obviously higher than that in group 2 and the control（F=39.43,q=9.72-11.87,P0.01）,that in group 2 was a little higher than the control,but with no statistical significance.Positive correlations were showed between serum levels of PAPP-A and CRP,and MMP-9 in patients of group 1 and group 3（r=0.424-0.472,P0.01）.Conclusion The rise of serum PAPP-A is related with inflammatory mechanism,which may indicate that the atherosclerotic plaques in head and neck arteries of the patients with AICVD are unstable.
