两种最小量外加压手术治疗孔源性视网膜脱离比较

摘　　要：目的比较自制医用双腔球囊巩膜外顶压手术与节段性巩膜外垫压手术治疗孔源性视网膜脱离(RRD)的效果。方法 RRD病人60例(60眼),A组38眼,采用自制双腔球囊巩膜外顶压术治疗;B组22眼,采用巩膜外节段性垫压术治疗。结果 A组32眼视网膜复位顺利,一次手术成功率为92.0%;术后视力提高28眼,不变6眼,下降4眼;出现并发症17眼。B组20眼视网膜一次手术复位,一次手术成功率为90.9%;术后视力提高16眼,不变3眼,下降3眼;出现并发症9眼。两组一次手术成功率、术后视力提高情况、并发症发生率比较,差异无显著性(P>0.05)。结论最小量外加压手术治疗RRD精确、可靠,创伤小,并发症少,手术成功率高;其中操作更为简便的球囊手术值得优先推荐。Objective To compare the effect of surgery with episcleral pressure using self-made two-way balloon method and segmental scleral bucking surgery for rhegmatogenous retinal detachment（RRD）. Methods This study included 60 RRD patients（60 eyes）,who were assigned to group A（38 eyes）,treated by surgery with episcleral pressure using self-made two-way balloon;and group B（38 eyes）,segmental scleral bucking surgery. Results In group A,reattachment of retina was achieved in 32 eyes,success rate of one-time operation（SROTO） was 92.0%;vision improved in 28 eyes;not changed in six,and decreased vision in four,complications were noted in 17.In group B,SROTO was 90.9%,vision improved in 16 eyes,not changed in three,and lowered in three;complications recorded in nine.The differences between the two groups were not significant in terms of SROTO,posoperatve vision and complications（P〉0.05）. Conclusion Minimum amount of external pressure operation for RRD is precise,reliable,little wounded,less complications and a high success rate,and the simple balloon procedure is worthy to be recommended in particular.
