乳腺癌保乳术后切线照射17例疗效分析

摘　　要：目的观察早期乳腺癌保乳术后全乳腺切线照射治疗的临床毒副反应及近期疗效。方法对2005年9月~2010年9月本院收治的17例早期乳腺癌接受保乳手术的患者,实行全乳腺切线照射至60 Gy,瘤床电子线推量6 Gy,随访其局部复发及远处转移,并按RTOG标准评估放疗反应。结果保乳术后放疗后患者中位随访30.5个月,无局部复发及远处转移病例。放疗结束时发生1级急性皮肤反应16例,2级急性皮肤反应1例,所有患者在随访期间均未观察到心脏、肺的放射性毒副反应。结论乳腺癌保乳术后全乳腺切线照射治疗,临床毒副反应小,近期疗效较好。Aim To observe the efficacy and toxicity of breast tangential irradiation after breast conservative surgery.Methods Altogether 17 patients were treated with conservative surgery and whole breast radiotherapy from September 2005 to September 2010.Prescription dose was 60 Gy for clinical target volume with 6-MV X-ray,and boost dose was 6 Gy in the tumor bed with electron.We followed up the local recurrence and distant metastasis and observed the side effect of radiotherapy according to RTOG standard.Results All patients were followed up with the median period of 30.5 months.There was no one with local recurrence and distant metastasis.Grade 1 of acute radiation skin reaction was seen in 13 patients and grade 2 in 4 patients and there was not more than grade 2 of skin reaction for all the patients.Excellent or good cosmetic outcome was 100% during follow-up.Conclusion The clinical outcome of the whole breast tangential irradiation after conservative surgery is good and the side effect is mild.
