食管静脉曲张在无痛胃镜下套扎治疗临床分析

摘　　要：目的研究食管静脉曲张在无痛胃镜下进行套扎治疗的效果与安全性。方法 182例肝硬化患者,共进行267例次内镜下套扎治疗,将其分成2组,其中普通内镜组115例次,无痛胃镜组152例次,分别观察两组术前、术中、术后心率、血压、呼吸、血氧饱和度、患者术中不良反应、术中出血情况,术后调查患者对内镜下套扎治疗的耐受情况及医师对手术的满意度,无痛胃镜组观察术前及术后7天的肝功能及凝血酶原活动度变化。结果无痛胃镜组术中收缩压与术前相比下降9.5%(P<0.01),舒张压下降15.3%(P<0.01),均为一过性,术后即恢复;普通胃镜组术中收缩压与术前相比升高10.5%(P<0.01),舒张压升高13.45%(P<0.01),亦为一过性,术后即恢复;两组心率、呼吸、血氧饱和度无显著性差异(P>0.05);术中操作医师对食管蠕动及视野满意度无痛胃镜组明显高于普通胃镜组(P<0.01)。结论食管静脉套扎术在无痛胃镜下进行是一种安全、易被患者接受的治疗方法。Objective To compare the efficiency and safety of esophageal varices banding by painless endoscopy retrospectively.Method One hundred and eighty-two patients with esophageal varices due to liver cirrhosiswere were divided into two groups：group A （82 patients） was treated by ordinary endoscopy,and group B （100 patients） was treated by painless endoscopy.Breathing,heart rate,blood pressure,and blood oxygen saturation were observed during operation,and adverse reactions,intraoperative bleeding,postoperative feeling of patients,and doctor＇s satisfaction were also recorded.In group B,liver function and prothrombin activity were examined preoperatively and postoperatively.Results In group B,systolic blood pressure of patients was decreased by 9.5% and diastolic blood pressure by 15.3% compared with preoperation （P 0.01）.In group A,however,systolic blood pressure of patients was increased by10.5% and diastolic blood pressure by 13.45% compared with preoperation （P 0.01）.There was no significant difference with regard to heart rate,respiration,oxygen saturation between two groups （P 0.05）.The more satisfactory operating vision was obtained in group B than in group A （P 0.01）.Conclusion Endoscopic esophageal varices banding by painless technique is safe,and easy to be accepted by patients.
