摘 要:目的探讨红细胞压积(HCT)及降钙素原(PCT)在重症急性胰腺炎(SAP)早期识别中的临床价值。方法分析40例因急性胰腺炎入院患者24h内血清HCT及PCT水平,评价发病24h内HCT、PCT的变化与急性胰腺炎(AP)轻重程度的关系及其对轻重症分型的早期预测价值。其中SAP组14例,轻型急性胰腺炎组(MAP)26例。应用全自动血细胞分析仪测定HCT水平,胶体金法测定血清PCT水平。结果 SAP组中HCT和PCT水平高于MAP组,差异有显著性(P<0.05),HCT值(>40%)判断重症急性胰腺炎的敏感性、特异性分别为57.14%和73.07%,阳性预测值、阴性预测值分别为53.33%和76.00%。PCT值(>0.5ng/ml)判断SAP的敏感性、特异性分别为92.86%和88.46%,阳性预测值、阴性预测值分别为81.25%和95.83%。结论 HCT、PCT可作为SAP的早期识别指标。Objective To evaluate the clinical values of hematocrit(HCT) and procalcitonin(PCT) levels as early markers in severe acute pancreatitis(SAP).Methods A retrospective analysis of serum HCT and PCT within 24 hours after admission in 40 hospitalized patients with acute pancreatitis were done.The correlation between the changes of HCT levels and PCT levels within 24h and the severity of aute pancreatitis(AP) were evaluated.The value of early predicting mild and severe stage of acute pancreatitis were evaluated.Of which 14 cases in SAP group,26 cases in mild acute pancreatitis(MAP) group.An automatic blood cell analyzer was used to detect serum HCT levels.The colloidal gold immunoassay(GIA) was used to determine PCT levels. Results Compared to mild acute pancreatitis group,HCT and PCT levels in severe acute pancreatitis group were statistical significantly lower(P〈0.05).HCT levels(〉40%) in diagnosis of SAP has value of sensitivity(57.14%) and specificity(73.07%);the positive predictive value was 53.33% and the negative predictive value was 76.00%.PCT levels(〉0.5ng/ml) in diagnosis of SAP has value of sensitivity(92.86%) and specificity(88.46%);the positive predictive value was 81.25% and the negative predictive value was 95.83%.Conclusion HCT and PCT levels appear to be early markerrs of SAP.
