摘  要:目的探讨乳腺小叶上皮内瘤变MMP-9的表达及血管生成的特点。方法 89例乳腺小叶上皮内病变标本(低级别LIN 27例,中级别LIN 38例,高级别LIN 24例),用免疫组织化学方法检测乳腺小叶上皮内瘤变MMP-9的表达,以正常乳腺组织(22例)和乳腺癌组织(68例)作为阴性及阳性对照,同时用CD31作为抗体检测乳腺小叶上皮内瘤变基质的血管密度。结果在乳腺小叶上皮内瘤变MMP-9过表达和微血管密度的增高,病变级别越高,MMP-9的表达指数越高,中级别和高级别病变MMP-9的表达指数接近浸润性癌。乳腺小叶上皮内瘤变MMP-9的表达指数与基质微血管密度相关。结论在癌前病变的中级别和高级别乳腺小叶上皮内瘤变MMP-9的表达指数与微血管密度增高,已经为乳腺癌的发生准备了条件,有可能在肿瘤的演进过程中发挥作用。因此,对于小叶癌,MMP-9和基质中的微血管一样,有可能作为治疗和预防的靶点。Objective To investigate the matrix metalloproteinase 9(MMP9)and microvessel density(MVD) during the different morphological alterations of lobular intraepithelial neoplasia(LIN).Methods89 lobular intraepithelial neoplasia(27LIN grade 1,38 LIN grade 2,24LIN grade 3) were immunohistochemically investigated for the expression of MMP9,while microvessel density(MVD) was counted using the anti-CD31 antibody,with 22 normal breast tissue and 68 lobular carcinomas as negative and positive control.ResultsWe identified overexpression of matrix metalloproteinase 9 and elavated MVD in LIN relative to normal breast epithelium and stroma.Expression index increased in lobular intraepithelial neoplasia group from low to high,and the expression index was positive related with MVD.ConclusionAngiogenesis and up regulation of MMP-9 are observed in preinvasive breast lesions.This might imply for new therapeutic strategies using both anti-angiogenic factors and MMP-9 inhabitor in order to prevent the progression of LIN to invasive carcinoma.
