摘  要:目的:定量检测肝病患者外周血缺氧诱导因子-1α(HIF-1α)的表达水平,以探讨血HIF-1α浓度作为肝细胞性肝癌(HCC)诊断及预后监测的临床价值。方法:收集不同肝病患者外周血220份及正常健康对照外周血27份,以酶联免疫吸附法(ELISA)定量检测肝病患者血清HIF-1α表达水平,与AFP对照分析其临床意义。结果:在肝病发展过程中,血清HIF-1α表达呈梯度增加,HCC患者血HIF-1α表达显著高于良性肝病患者(P<0.001),与肿瘤大小、有无肝外转移明显相关(P<0.05),与患者的年龄、性别、AFP浓度及HBsAg阳性与否间差异无统计学意义(P>0.05)。受试者工作曲线显示,血HIF-1α面积水平为0.909,血HIF-1α表达水平异常,有助于HCC的诊断。结论:HIF-1α过表达与HCC的发生、发展相关,外周血HIF-1α异常是HCC诊断和转移监测的良好标志物。Objective: To investigate quantitatively the levels of circulating hypoxia-inducible factor-1α(HIF-1α) expression in a range of patients with liver diseases for exploring its diagnosis and monitoring prognosis for hepatocellular carcinoma(HCC).Methods: The levels of circulating HIF-1α were quantitatively detected in 220 patients with liver diseases and 27 health controls by ELISA,and analyzed clinical values comparative with serum AFP level.Results: The levels of serum HIF-1α expression were increasing during the development of liver diseases.The expression of circulating HIF-1α in HCC patients was significantly more than patients with acute or chronic hepatitis,liver cirrhosis,and normal controls(P0.001) without significant different between HIF-1α expression and age,sex,AFP and positive-HBsAg(P0.05) except of tumor size and extra-hepatic metastasis(P0.05).The area under receive operating characteristic curve of HIF-1α expression was 0.909 and indicated that the abnormality of serum HIF-1α level could be a useful to HCC diagnosis.Conclusion: HIF-1α over-expression is associated with occurrence and development of HCC,and circulating HIF-1α level is a useful molecular marker in HCC diagnosis and monitor prognosis.
