摘 要:目的:探讨食管癌贲门癌患者术后早期肠内营养治疗的效果、安全性及可行性。方法:255例老年食管癌、贲门癌患者随机分成肠内营养组(EN组,130例)和肠外营养组(PN组,125例),分别监测手术前后体重、白蛋白、前白蛋白、血红蛋白,观察术后肛门恢复排气排便时间,肺部感染、术野感染、心律失常、心衰发生情况并作对照分析。结果:EN组在营养支持治疗后与PN组比较,肛门恢复排气排便时间明显缩短,体重、白蛋白、前白蛋白、血红蛋白明显升高(P<0.01),并发肺部感染、术野感染、心律失常、心衰等明显减少(P<0.01)。结论:老年食管癌、贲门癌术后早期肠内营养治疗能有效改善机体营养状况,提高免疫能力,促进肠道功能恢复,减少感染,积极预防心肺并发症,值得临床推广。Objective: To investigate the curatrive effect,safety and feasibility of early enteral nutrition after esophageal and cardiac carcinoma surgery.Methods: 255 elderly patients with esophageal and cardiac carcinoma were randomly divided into enteral nutrition group(EN group,130 cases) and parenteral nutrition group(PN group,125 cases).Their weight,albumin,prealbumin and hemoglobin were monitored before and after surgery respectively.After operation,their defecation and exhaust time,pulmonary infection,surgical field infection,arrhythmia and heart failure were observed.Results: Compared with those in PN group,in the EN group after treatment with nutritional support the time of defecation and exhaust was significantly shorter,the weight,albumin,prealbumin and hemoglobin were significantly higher,but the pulmonary infection,surgical field infection,arrhythmia and heart failure were significantly reduced(P0.01).Conclusion: Early postoperative enteral nutrition therapy can improve the nutritional status,immunity and the recovery of bowel function in the elderly patients with esophageal,gastric cardiac carcinoma.It also can reduce the infection and prevent the cardiopulmonary complications.EN is worthy to be spread.
