3种手术方式治疗非小细胞肺癌的临床研究

摘　　要：目的:探讨非小细胞肺癌全胸腔镜下根治术与胸腔镜辅助小切口、常规进胸根治术的治疗方法和效果。方法:本组1035例研究对象均为I～II期非小细胞肺癌患者,分别以3种不同手术方式行肺癌根治术,并对其临床资料进行回顾性分析。1035例肺癌根治术中,637例常规进胸,256例胸腔镜辅助小切口,142例全胸腔镜下手术。结果:本组1 035例均获成功,93例出现术后肺部并发症,37例出现心血管并发症,905例均未出现术后并发症。全组3种手术方法淋巴结均分组规范清扫,预后均无显著差异;全胸腔镜肺癌根治术中、术后出血少,术后并发症少,住院时间短,免疫力恢复较快。结论:全胸腔镜肺癌根治术对肺癌是一种完全可行并且安全的手术方法,具有创伤小、出血少、恢复快、切口美观等优点,全胸腔镜手还具有术后并发症少、免疫力恢复较快的优点,因此全胸腔镜手术有可能在肺癌手术中得到更广泛的应用。Objective： To evaluate the treatment effect of radical resection of pulmonary carcinoma by video-thoractomy,video-as- sisted mini-thoractomy（VAMT）.and conventional thoracotomy on non-small cell lung cancer（NSCLC）. Methods：The clinical material of i 035 cases with I ~ 11 stage NSCLC underwent, radical resection by three different surgery ways were reviewed, six hundred thirty -seven patients were taken by conventional thoracotomy, 256 patients were taken by video-assisted mini-thoractomy and 142 patients were taken by video-thoractomy. Results：All 1 035 operations were successful. Pulmonary complication occurred in 93 cases,and cardiovascular complication occurred in 37 cases. No significant post-operative complication was found in other 905 cases. All the cases were undergone grouping standard lymph node cleaning, and the prognosis had no significant difference. The radical resec- tion of pulmonary carcinoma by video-thoractomy had the advantages of trauma, less complication, shorter in-hospital time, rapid immunity recovery. Concluslon：The radical resection of pulmonary carcinoma by video-thoractomy is completely feasible and safe. It has the advantages of less trauma and hemorrhage, rapid recovery, artistic margin and so on. It also has less complication and rapid immuni- ty recovery. Therefore the video-thoractomy has the possibility to obtain a more widespread application in the lung cancer surgery.
